2 Aim  To equip staff with the knowledge and skills to accurately complete and introduce the Nutritional Care Communication tool  To ensure safe, effective communication between hospital and care homes in relation to nutritional care.

3 Learning Outcomes  Outline the prevalence of malnutrition nationally.  Discuss the importance of effective communication between hospitals and care homes.  Outline the nutritional care communication tool to ensure consistent and safe approach to patient journey.

4 Background and context  1 in 3 of all patients of all ages admitted to hospital and care homes are at risk of malnutrition.  Overall prevalence of malnutrition on admission to care homes in UK 30-40% (BAPEN 2010)  NHS QIS National overview feedback  Phase 2 Programme – focus on frail elderly and discharge planning

5 Example from practise: September 2010  Mrs X came in for respite. ‘MUST’ recorded and care plan implemented: food recording, weekly weight and fortified diet.  Mrs X went home to prepare for coming in long term, family aware of nutritional care plan.  Admitted to hospital not eating. In hospital for 1 week, returned to care home, No mention from Hospital of poor diet, concerns regarding weight or evidence of ‘MUST’ recording. Seen by SALT but not referred to dietitian.

9 Way Forward  Local spread plan  Complete 1 form for patient/resident.  Photocopy – reserve copy for collection.  Original copy with patient/resident.  Contact local lead.  Follow up contact - feedback
