5 OPM Online Services-Use it to:  Change Federal & State Income Tax Withholding  Change Mailing Address  Change Password  Establish an Allotment to an Organization  Set up Checking or Savings Allotment  Sign up for Direct Deposit

6  Update email Address & Opt-in to Receive Information Electronically  View/Print Annuity Statement/Verification of Income  View/Print 1099-R Tax Forms  View/Print Verification of Life Insurance (FEGLI)  View Status of Retirement While in Interim Pay OPM Online Services-Use it to:

7  Survivors, Family, or Estate Representatives are Required to Notify OPM in the Event of the Benefit Recipient's Death  Life Events, such as Marriage/ Divorce, Death of Spouse or Reemployment can affect benefits and must be Reported to OPM Immediately  Services Online cannot be Used to Make Changes to Federal Dental/Vision Insurance (FEDVIP) Call BENEFEDSCall BENEFEDS or call 1-877-888-3337 for Changes/Comments Related to FEDVIP Coverage OPM Online Services

8  Each Annuitant or Survivor must have a CS number to use on line services- enter nine characters (retired employee, the claim number begins with an “A” and ends with a zero - “0; a surviving spouse, the claim number begins with an “F” and ends with a “W” )  Each Account has a Password (if you need to reset the password, it can take a few days)  If you are a Representative Payee, Guardian, Conservator, or any other Third Party Representing an OPM Annuitant you cannot use Services Online OPM Online Services

9 Reporting Annuitant’s Death to OPM Online  Must Enter -Deceased's Civil Service Annuity Claim Number or Social Security Number  Other Required Fields: – Name of Deceased, Date of Birth, & Date of Death – Surviving Spouse Information: Name, Telephone #, & email (Address Optional) – Informant Information: Name & Phone #  No Password Required  Active Federal Employee Deaths are Reported by the Agency where they were Employed at the Time of Death

10 What the Survivor Can Expect Next  Upon Notification of Death- OPM will Stop Further Payments, Create a Survivor File, & Assign a CSF Number  OPM will Send a Bar-Coded Death Form or a Survivor Express Form to be Completed in 30 days  OPM will also Send Claim Form for FEGLI Benefits  The Bank for Direct Deposit must be Notified Not to Accept any Further Payments  OPM will also Change Health Coverage, if necessary, to Self-only

12 Social Security Office-What to Bring  Proof of death (either from funeral home or death certificate)  Social Security number, and the deceased family member's number  Your birth certificate  Your marriage certificate if you are the widow or widower  Proof of U.S. citizenship or lawful alien status if you were born outside the United States

13  Your divorce papers if you are applying as a surviving divorced spouse  Dependent children's Social Security numbers and birth certificates, if available  Deceased worker's most recent W-2 forms or federal self-employment tax return  The name of your bank & your account number so your benefits can be directly deposited into your account Social Security Office-What to Bring
