5 Data: What we will be providing Provider-level: Individual clinicians within a practice (on QI TeamSpace, including comparison to clinic aggregate – Reports) Practice-level: Aggregate of all clinicians participating within the practice, including comparison to project aggregate (on QI TeamSpace – Reports) Project-level: Your practice as it compares to other practices in the project (on Team Lead Calls) Keep in mind that not all providers had an N of 30 but we are reporting percentages. See your report for specific ‘N’

6 Percentage of encounters for initial diagnosis and follow-up for ADHD where there is documentation of the screening tool in 2 settings documented in the chart

7 Percentage of encounters for Initial Diagnosis where there is documentation of DSM-V criteria and Co-Existing Conditions

8 Percentage of encounters for Initial Diagnosis and ADHD follow- up where there is documentation of medication follow-up within the recommended time of 30-45 days for new Rx and 6 months for maintenance Rx

9 Optional Measures: Follow-up documentation and ADHD Registry
