1 Abdominal Contouring After Massive Weight Loss Objectives (Aims): Morbid obesity is an increasing problem of all countries in the twenty-first century. Worldwide, more than one billion adults are overweight. Medical therapy for morbid obesity has limited short- and long-term success. Bariatric surgery has been proven to be the only effective long-term treatment for morbid obesity. Massive weight loss (MWL) leaves a patient with loose, hanging skin on the abdominal wall, thighs, arms, buttocks and flattened ptotic breast. Subjects and Methods Thiry-four female (26 post- bariatric patients) and 7 male (5 post-bariatric patients) were operated for abdominal deformity in last nine years. Mean age was 34.6years (range 19 – 57 years). Results Mean body mass index was 28.4 at the time of the abdominal contouring surgery. Twenty-four circumferential lower body lift and 17 abdominoplasty (7 Fleur-de-lis abdominoplasty) were performed. Additional operative procedures with abdominal contouring procedures were 12 autologous gluteal augmentation, 8 mastopexy or/and breast implant and 7 liposuctions. Complications were 8 delayed wound healing, 2 hematoma, 3 seroma and 1 atelectasia, 1arrythmia, 1 deep vein thrombosis. Three patients were treated in the intensive care unit. Twenty-two patients were transfused with autologous blood transfusion. Mean hospitalization duration was 4,2 days. The lower body lift is an optimal first step procedure that allows simultaneous skin resection and tightening of different regions, including the abdomen, buttocks, hips and thighs. A frequently in patients after massive weight loss observed mons pubis hypertrophy and ptosis is challenged through three-dimensional monsplasty. Circumferential procedures, take advantage of the architecture of the superficial fascial system, which allows the forces created during inferior flap suspension to be dispersed to the adjacent subunits of the trunk, elevating the buttock and thighs simultaneously.
