2. For First year student-athletes and Transfers:
For returning student-athletes, check your noctrl.edu for Athlete ID and Password OR click Forgot your username or password button and the information will be ed to you again.
For First year student-athletes and Transfers:
Athlete ID: new and Password: new

3. Fill-in all yellow highlighted fields on this screen

4. Save Athlete information and move to the Insurance tab above
You must fill in the medical alerts and Allergies box (if this does not apply, write None in each box)
Save Athlete information and move to the Insurance tab above

5. Fill in all yellow fields and click Save Athlete Information, the proceed to the Contacts tab to provide emergency contact information.

6. Fill-in all yellow fields and click Save Athlete Information
Fill-in all yellow fields and click Save Athlete Information! Once you have completely filled in this information, new tabs will appear at the top of your screen.

7. Next, go to the Forms tab (no information needs to be provided under Paperwork tab)
Select the Assumption of Risk form and click New

8. Read Statement above, E-Sign, type in your name, click Sign and Save

9. Select CCIW Injury Reporting Form, click New

10. Read through the statement, E-sign, type your name, click Sign and Save

11. Next, select Consent to Treat and click New

12. Read the statement, E-sign, type in your name and click Sign and Save.

13. Click on the next form, Mental Health Questionnaire (this is optional) and select new.

14. Optional: select the most appropriate answer for yourself, e-sign, type your name and click Sign

15. The next form is required: Medical History
The next form is required: Medical History. Select the form and click new.

16. Note: This is a 3 page document

18. Once you have completed the entire form, E-sign, type your name and click Sign and Save.

19. The final form to complete is Sickle Cell Trait Testing

20. Read through the form and answer each question
Read through the form and answer each question. If you have the test results, please upload the results to the E-Files page. No supporting documentation is required if you select to waive the Sickle Cell test. E-sign and click Sign and Save.

21. Please put your last name and 2016-17 Physical as the Description.
Under the eFiles Tab, you can verify that all forms have been submitted. If any of the forms are missing, go back to the Forms tab and re-submit any missing documents.
The final step allows you to upload a copy of your physical form that has been signed by a physician (MD or DO) and dated after June 1, Use a scanner to save the physical on your desktop, then, choose the scanned physical from your desktop and click Upload.
Please put your last name and Physical as the Description.
