The University of Utah College of Social Work HPPAE: Context The University of Utah (UU) College of Social Work (CSW) launched a service arm in 1997, the Neighbors Helping Neighbors Program (NHN), to help low-income community-dwelling adults, age 65+, remain independent in their homes and retain their quality of life.

11. UU CSW HPPAE: History and Evolution  NHN was funded as a HPPAE site by the JAHF 2005-2008.  2 nd year MSW students completing their practicums at NHN receive a stipend and tuition waiver (then & now).  In addition to fulfilling basic 2 nd year MSW practicum requirements in NHN, HPPAE students (then & now):  Facilitate Vital Aging Groups in senior centers—Fall semester and Caring Connections Grief Groups—Spring semester  Complete a rotation in a community agency serving adults, age 65+, such as hospice, adult day services, nursing home, local/state government, VA Medical Center  As the primary field instructors, LCSWs at NHN provide weekly group and individual supervision and coordinate students’ learning experiences with community agency instructors and task supervisors (then & now). –

12. UU CSW HPPAE: Sustainability and Innovation  We continued HPPAE after JAHF funding ended.  We promote the NHN practicum and the Aging Concentration on the CSW website, in MSW application materials, at concentration selection workshops, at practicum fair, and at promotional activities with food.  Our College supports staff and faculty costs associated with the NHN program.  Support for NHN student stipends and client service costs comes from community collaborations and private funders.  We submitted a funding request to the UT Legislature this year to support and expand NHN. Our request wasn’t funded but two Senators championed our cause and plan to resubmit in 2016.

13. UU CSW HPPAE: Going Strong in 2015  More than 2,700 individuals have been served by our students and community partners since the program began.  Last year, 250 adults 65+ (including 70 couples) received direct care, home repair assistance, and/or referral services.  Funding for stipends from local private foundations helps us be competitive.  We focus on advanced practice gero social work competencies and use the Geriatric Social Work Scale II for pre–post evaluation.  New leadership at the college, at the WD Goodwill Initiatives on Aging, and at NHN as of July 1, 2015, is committed to sustaining and our growing HPPAE Program.,

14. UU CSW HPPAE: Paying It Forward… Expanding HPPAE to VA/Geriatric Research Education and Clinical Centers (GRECCs)  The JAHF, the VA, the SWLI, and the CSWE Gero-Ed Center partnered to expand HPPAE to VA/GRECCs 2012–2015.  Cohort 1 included Salt Lake City, UT, Madison, WI, and Birmingham, AL. These sites were selected to pilot the model in 2012. Fran Wilby and I served as local mentors to the SLC VA/GRECC HPPAE, which is in its 4 th year.  As a national mentor for the VA/GRECC HPPAE collaboration 2012–2015, I have delivered presentations at site-specific orientations and provided phone support to the 15 sites that comprise Cohorts 1-4.
