1 “HONORING THOSE WHO SERVED” ENSURING ACCESS TO HOSPICE FOR OUR VETERANS

2 LHC Group Hospice Division Access to Hospice Care for Veterans 28% of Americans who die each year are veterans. Over 1800 veterans die each day Veterans have special care needs at the end of life, especially if they are combat veterans It is imperative that hospices step up, acquire the necessary skills and serve these veterans with the dignity they deserve.

3 WHAT DOES WE HONOR VETERANS MEAN?? Asking about military history and knowing what to do with the answers Partnering to design care specific to veterans needs. Extending VA and agency “reach” to improve care and access. Improving quality by measuring the impact of VA and agency interventions.

4 MISSION Providing the highest quality of care to veterans diagnosed with terminal illnesses in the best possible manner and setting while honoring the veterans preferences.

5 GOALS Focus on the needs, goals and desired treatment of veterans Enhance partnerships with area veterans healthcare and community resources Develop education resources for veterans, the VA and community physicians Align veterans with entitled benefits

6 ROLL-OUT TIMELINE 11/10/10 Compile list of Veteran opportunities in Fayetteville, NC and throughout the state of Arkansas where we have agencies that include VA medical centers, Veterans Homes, VA CBOCs, Dept. of Veterans Affairs, VFW and American Legions. 11/15/10 Assemble “task force”. Includes: Mary Briles, Shelley Denton, Richard Carlini, Kathleen Guidry, and Cindy Wells. Meetings will be planned every other week until program toolkit is completed from NHPCO. 11/18/10 Initial Program Call with assignments, roll-out timeline and program objectives following the “We Honor Vets” toolkit. 11/22/10 Military Checklist developed to help with our assessments of our veterans needs.

7 Roll-out Timeline Con’t 12/9/10 In-service Developed and ready for the task force to review. 12/10/10 Items ready for order to include: Veterans Planning Guides, Promo items, Military Assessments ( Banking Forms vs Word ?)Flag Pins, Vet to Vet info brochures etc. Staff education to be reviewed by task force 12/17/10 Rollout call to start with Arkansas agencies, Jennings and Fayetteville administrators and PCRs with education on program 12/21/10 Review of education Material for families by task force. January 2011 Identified agencies will launch program

8 STEPS TO DEVELOPING THE PROGRAM Research Identify Outreach opportunities Assemble “Task Force”/ Create Timeline/ Assign duties Develop Military Checklist and In-Service Materials Purchase Necessary Supplies Develop Training Materials for Staff Conduct Staff Training Develop Educational Materials for Families Promotion Materials/ Recruit Veterans to use as volunteers Launch “ We Honor Veterans” Evaluate program Military
