2 Context Public Sector reforms - reducing resources. Implementation of the Care Act and new duties on Local Authorities. Health & Social Care integration. Increasing demand on Health & Social Care services as more people live longer with long term health conditions.

3 Care Act 2014 Greater emphasis on delivering service user outcomes and high quality services Duty of Well Being and to provide/arrange preventative services Improve information and advice New duty to undertake carers assessments and provide services that help support carers Market Shaping responsibility which means the council has to understand and develop all parts of the market including for self funders

4 Community Capacity To support the management of demand and the prevention agenda we aim to: Increase the capacity of the community & voluntary sector organisations and other cross sector services. To ensure that a process is developed to identify gaps in service that need to be addressed with new solutions. To support the improvement of information and the collation of services onto the Local Directory so that citizens and professionals are able to identify appropriate interventions to engage people and prevent, reduce or delay an escalation of their needs.

5 Community Capacity building In a climate of growing demand and decreasing resources, the need to effectively target support to those most at risk of needing services in the future becomes even more critical. The outcomes that need to be achieved will include short, medium and long term plans to effectively meet a wide range of universal, moderate and eligible needs across Bolton. Working with children and adults across diverse communities will require a whole system approach with partners from Housing, Faith, Health, Education, Sport, Leisure, Community and Voluntary, Private and Public Sectors working alongside the citizen’s of Bolton who have a lot of assets to offer. Building relationships across sectors and agreeing shared outcomes through the development of projects, networks,forums and strategies will help maximise the use of our current resources and put Bolton in a stronger position to help people help themselves before their needs escalate. With citizens that we support accessing housing, health and a number of other services independently this is everybody's business and a key part of my role is to connect these stakeholders together where there is common interest so that they can work together to increase capacity and improve outcomes by making better use of existing resources.

6 The Marketplace The existing offer to prevent, reduce and delay the need for Health & Social Care services supports people with a range of low to moderate needs and in some cases complex needs alongside statutory services. These include support for Children, Adults and Older people and cover a vast range of topics: Mental Health Disability Health conditions Age related conditions Carers Isolation Employment Education Information & Advice Advocacy & Brokerage

7 Examples of services Lunch Clubs, befriending, social activities provided by Age UK. Senior Solutions, Asian Elders and Halliwell befriending. Hydrotherapy sessions at Bolton one provided by Bolton Neuro Voices. Stroke support groups – Bolton Stroke association, Jigsaw and Bolton west stroke group, Bolton Carers support Bolton Dementia support Universal services

10 What this could mean for you? A resource in the council that you can utilise to develop new services or increase capacity across the universal and preventative provider market. Support to identify and facilitate cross sector collaboration opportunities that deliver shared outcomes. Somewhere to suggest improvements to the provider marketplace and to raise concerns about unmet need and demand. Somewhere to connect your knowledge of local projects, organisations or assets into a central point for all to share. Someone to share ideas with that could improve outcomes for Bolton citizens.

11 Vision going forward Cross sector providers from Housing, Faith, Voluntary and Community Sector, Health and Social Care, Emergency Services, Private Sector, Education, Employment and Leisure Services working together to empower individuals and communities to meet their own needs through an asset based approach with organisations supporting from behind and not creating dependencies. This will involve pooling or aligning resources to train and recruit volunteers, engage citizens to help them selves and to remove barriers that appear.
