Dione Rogers – Head of Capacity and Patient Flow - Ext 4316 Please ensure that for these patients you check: Keys to access patient’s residence Understanding of medication regime Transport arrangements Relatives/carers are aware, and know what to do Our new ‘Discharge to Assess’ process means patients who need ongoing health/social support will have a detailed assessment of their needs in their own home, enabling an earlier discharge from hospital All patients requiring health/social support on discharge must have a ‘Patient Needs Assessment Form’ (PDNA) completed once medically fit Prepare patients to go to the Discharge Suite on the day of discharge so they can be ‘home for lunch’ See the intranet homepage for more details
