2 MISSION STATEMENT To provide preventative care and improve the health of the underserved migrant populations by providing high quality, accessible health care to the Janesville community and surrounding areas which is affordable.

3 PHILOSOPHY Our philosophy is based on Watson’s Philosophy and Science of Caring model as well as following the Person-Centered nursing framework. Delivers patient care by using evidence based practice and a multi- disciplinary approach Respectful towards diverse populations and is culturally knowledgeable Aims to achieve optimal health, physically and mentally Provides convenient hours and accessibility to all clients Provides competent, family oriented care

4 Goals Provide thorough education for both patients and families To improve support programs for migrants To provide up-to-date standards of care To have compassionate employees To be accessible to migrants unable to travel to the clinic by having a mobile clinic

7 Job Descriptions Nurse Practitioner: Advance practice RNs who work in collaboration with other HCP to deliver focused care. Must have a graduate level education and clinical training in Family Medicine. Registered Nurse: Facilitate team members to ensure smooth operations. Coordinates patient care and directs efficient clinic flow. Six months or more clinical experience. Experience with Epic software preferred. Social Worker: Will fulfill many roles such as care management, psychotherapist, and community outreach coordinators. Should be able to identify psychosocial problems. Bachelors with clinical experience; Masters preferred. Medical Assistant: Will fulfill dual roles in clerical and clinical work. Qualifications include clinical skills, health promotion, medical teamwork, and verbal communication. Laboratory Technician: Provide information for patient diagnoses and treatment by performing laboratory tests in toxicology, hematology, immunology, and microbiology labs. Qualifications include infection control, documentation, analyzing information, and communication results. Volunteers: Nursing, Social Work, Medical Assistant, Laboratory Technician, and Medical students and professionals. Follow directions from staff. Gain hands-on experience, and fulfill volunteer hours working with clients and various cases.
