It's an online store stocked with a wide variety of products and programs from which you choose what you need. Enrolling in Bright Choices is like nothing you’ve ever done before. It’s a completely revolutionary way to shop for benefits… Bright Choices will recommend a unique benefits portfolio just for you. Life's uncertain... but, we can protect ourselves.

3. 3 BRIGHT CHOICES OVERVIEW, CONTINUED You don't have to take Bright Choices' recommendations. You're free to elect any plans you want... but there's a lot going on in the background in Bright Choices to help you find the best plans. Let the system work for you. Bright Choices uses your benefits "personality" — everything it knows about you — your age, gender, marital status, family size, health background, and concerns and needs — to find the best plans to protect your health and your wallet.

4. 4 BRIGHT CHOICES, “AT A GLANCE” Private online benefits store for each employee. Employees shop for benefits from a range of choices, including health, money, and protection products. 5. 5 LOGGING IN http://exchange.liazon.com –Login Name: Email Address –Password: Your password is provided to you via email.

6. 6 HOME PAGE After logging into the portal, you’ll be greeted by the Home Screen. Click on to begin your benefit selection process.

7. 7 INTRODUCTION On the “Introduction” page, you’ll find educational materials and instructions, including a look at the plans available to you before you get started. Click to start shopping for benefits.

8. 8 SHOP FOR BENEFITS In “My Profile,” you’ll answer questions about your lifestyle, dependents, and risk tolerance. 9. 9 PROFILE – CONTACT INFORMATION You can also review your personal information and update your email address and telephone numbers. Select to proceed.

10. 10 PROFILE – MY DEPENDENTS Here, you can add any dependents, such as a spouse or your children. Select to proceed.

11. 11 PROFILE – MY HEALTH Complete the Quick Health Profile for yourself and dependent(s) by clicking on the blue box for each individual.

12. 12 PROFILE – MY HEALTH Answer the Quick Health Profile questions for the individual selected, then click the button at the bottom of the page.

13. 13 PROFILE – QUESTIONS You’ll also be asked to answer additional, brief questions that will further tailor your recommended benefits portfolio.

14. 14 PROFILE RESULTS Once your profile is complete, you’ll review the factors on which your recommendations will be based. Click to view your recommended benefits portfolio.

15. 15 SHOP FOR BENEFITS – RECOMMENDED PORTFOLIO Keep in mind that our recommended benefits portfolio is calculated based on your responses, but this is only a recommendation. You’ll select and enroll in the benefits you choose in the following step. Clicking any of the links under the Plan/Program or Find Out Why columns provides additional information for each benefit.

16. 16 SHOP FOR BENEFITS – RECOMMENDED PORTFOLIO, CONTINUED At the bottom of your recommended benefits portfolio, select to start with your recommended benefits or to start with an empty cart.

17. 17 ENROLLING IN YOUR BENEFITS – SELECTING BENEFIT PLANS Choices from your recommended portfolio are denoted by the “Recommended” icon. To view additional plan options select.

18. 18 ENROLLING IN YOUR BENEFITS – SELECTING BENEFIT PLANS, CONTINUED You can make any coverage selection you desire by clicking the button next to the plan you want to edit.

19. 19 ENROLLING IN YOUR BENEFITS - SELECTING BENEFIT PLANS, CONTINUED Any unavailable or automatically-assigned benefits are indicated by the icon. Once you’ve customized your Portfolio, click the button to enroll in all of your benefits.

20. 20 ENROLLING IN YOUR BENEFITS – LOCKING IN YOUR SELECTIONS To confirm and finalize your benefits enrollment, place a check in the acknowledgement box; then click the button.

21. 21 LIFE EVENTS IN BRIGHT CHOICES Things change in life – and often those changes can affect your benefits. Things such as: Marriage Divorce Birth Adoption Change in Employment Death of a Spouse or Child You can initiate your changes in the Things Change section of Bright Choices.

22. 22 THINGS CHANGE IN BRIGHT CHOICES Things Change is located on your post-enrollment homepage. If you have a life events request after you’ve enrolled in your benefits, you’ll go here to submit it. Click on the “Change Requests & Updates” or the button to continue.

23. 23 SUBMITTING A LIFE EVENT REQUEST Click the button to get started. Bright Choices will guide you step-by-step through the process. You’re prompted to select a family status change, presented with plan options and rates, and asked to review and select changes.
