2 NHS West Kent Clinical Commissioning Group What is urgent care? Urgent Care is the range of responses that health and care services provide to people who require urgent advice, care, treatment or diagnosis. People using services and carers should expect 24/7 consistent and rigorous assessment of the urgency of their care need and an appropriate and prompt response to that need. 2

4 NHS West Kent Clinical Commissioning Group What does this mean for patients? 1. Improving access to the urgent care system & improving assessment of patient’s medical & social care needs We aim to reduce delays and ensure the right treatment is available, at the right place and at the right time through improved assessment of patients and easier access to services Integrated Clinical Assessment and Management Service Single point of access for mental health services 2. Improving patient outcomes in out-of-hospital care Multidisciplinary teams will work across both primary care and community services ensuring patients receive specialist urgent care treatment closer to home Locally based community teams & more flexible community hospital provision Commissioning of two Urgent Care Centres Therapy service providing continuity of care across acute and community 4

Improving the quality and patient experience of acute hospital care All patients attending A&E assessed by a senior clinician within one hour Improved early warning systems for high risk patients For patients who require admission we will work towards ensuring there are no delays to their discharge Commissioning of a frailty unit (specialist ward for frail older people) whilst also ensuring Clinical Decision Unit & Medical Assessment Unit (short stay wards) are used effectively and efficiently 24/7 mental health liaison service in A&E Improving joint working with community and primary care to facilitate speedier discharges & joint care planning 5

Focusing on self-care and prevention to reduce acute admissions We will improve the access to primary care and will support patients to manage their long term conditions more effectively. Patients will be treated closer to home. Roll out of the West Kent frailty pathway (emphasis on pro-active health management) Developing ambulatory care pathways in the community Supporting patients to self-management of illnesses Reducing the demand for unscheduled care by more effective planned and elective care. 5. Delivering a system wide approach to urgent care Working as an Urgent and Emergency Care Network, rather than individual services. This will make it easier for patients to navigate the system and will increase the numbers of patients treated closer to home; as well as improving patient satisfaction. Improved technology to manage demand and seasonal pressures Workforce development Joint commissioning of services with other CCGs and Kent County Council 6

7 NHS West Kent Clinical Commissioning Group Engagement and Strategy Development Timeline December 2015 to February 2016 - draft strategy developed February - engagement: –PPG Chairs discussion and feedback –Promote summary of draft strategy with questions for feedback March – review feedback and comments End of April – finalise strategy May through July – launch of the strategy, including further engagement with GPs, patients and the Public 7
