4 CFS has replaced the traditional manual entry of Change of Address (COA) cards though the COA Forms Processing System (CFPS). COAs are scanned using a high speed scanner. ENTER

5 Images are captured and verified for accuracy and completeness. ENTER Ensure the customer has signed and dated the COA form.

6 Once the COAs are scanned and verified for accuracy, labels are printed for each ZIP Code and sent to the delivery units. ENTER

7 Upon delivery unit receipt, the labels must be dispensed to the appropriate carrier or box clerk. Labels are in Carrier Route order. ENTER

8 Once the PS Form 3982 label is returned, review the label for accuracy and ensure that the following information is correct:  Spelling of customer’s name  OLD address information  Move type – Individual, Family, Business  Start Date  Action: This will provide you information of the type of transaction in the Change of Address database – ADD, MODIFY, DELETE  End Date: This is the Temporary Order end date.  Note: The label does not contain the new address information ENTER

10 If all the information on the PS Form 3982 label is correct, the label is applied to a new PS Form 3982X (Carrier COA record). The form has been revised to accommodate the size of the new PS Form 3982 label. The revised PS Form 3982X will hold 14 labels. ENTER

11 The following slides explain how to utilize the new PS Form 3546 in order to change or correct 3982 labels that contain erroneous information. ENTER

12 PS Form 3546 is the official Postal Service document used for initiating a change or correction to an existing Change of Address (COA) record. ENTER The new COA Forms Processing System (CFPS) has created the need to redesign the PS Form 3546 in order to accommodate the application of the 3982 label and to facilitate changes to existing COA records.

13 Changes to the PS Form 3546 include the following: ENTER  Section 1 - An Action block has been added in order to assign the type of change.

14 ENTER  Section 2 - A block to affix the 3982 label or to write the customer information.

15 ENTER  Sections 3 & 4 - Blocks to change the spelling of the customer’s name or business name.

16 ENTER  A block to change the end date on a temporary change order.

17 ENTER  Section 5 - A block to change the old address information.

18 ENTER  Section 6 - A block to change the new address information.

19  A Block for employee initials, current date and Route ID number. ENTER

20 The following slides explain how to properly complete each block. Place a check next to each action requiring correction. In this example, the Correct Name action has been indicated. ENTER

21 Apply the PS Form 3982 label containing incorrect information to the PS Form 3546 in the block provided. Write the customer’s correct name in blocks 3 & 4. 22 Do not write on, make changes to or otherwise obliterate any portion of the 3982 label. Critical information contained on the label is necessary in order to match the original order. ENTER Make all 3982 changes and corrections on the PS Form 3546.

23 Preparing a PS Form 3546 without a PS Form 3982 label: ENTER If the PS Form 3982 label is not used, the following information must be manually entered in writing: Move Type – (Individual, Family, Business) Customer’s Name Customer’s Old Address

24 To correct the ORIGINAL order, ALL information in this block MUST match the ORIGINAL order of record EXACTLY. HOWARD, WALTER W ANYWHERE, CA 900000001 1234 ANY WAY In this example, the original order was incorrectly entered with the last name Howard; therefore, the name must be written as “HOWARD”. ENTER

Notice the last name was changed from Howard to Poward Complete the PS Form 3546 by Initialing, dating and entering Route ID Number. LH 9/01/05 C21 ENTER If you don’t have access to the 3982 label, do not use a CFS or CIOSS label off a mail piece as a replacement. Enter the old address manually exactly as it appeared on the 3982.

26 LH 9/01/05 C21 To change an order from Individual to Family or visa versa check the appropriate Action box. In this example, the order is being changed from an Individual order to a Family order. Ensure the PS Form 3546 is initialed and dated. ENTER

35 To change an existing forwarding order to a Moved – Left No Address (MLNA) or Box Closed No Order (BCNO), apply the 3982 label to Section 2 and check Correct New Address in the Action Box. Write “Moved Left No Address” or Box Closed No Order” in Section 6. LH 9/01/05 C21 ENTER Moved Left No Address

36 CFS will return to the delivery unit a new 3982 label bearing the corrected information. Apply the label to the 3982X (Carrier COA Record). ENTER

37 Why doesn’t the 3546 have an indicia or a place to write the Postmaster’s address? The PS Form 3546 is an internal form. It should not be handed to the customer for completion and it should not be placed in the mail stream. How do Window Clerks help customers fix an error they made on their COA? Do not give them a PS Form 3546. Give the customer a PS Form 3575 and have them fill it out completely. The carrier or box clerk must fill out and submit PS Form 3546 with the appropriate action box checked in Section 1. The existing information will be updated as specified by the customer. How do you complete the new PS Form 3546 for a customer going from address A to address B to address C? Place the 3982 label or print the old (A) address in section 2. Then print the newest (C) address in section 6 and check the Correct New Address action box in section 1. Do I have to complete Section 2 on every PS Form 3546 I fill out? No matter what action you are trying to take, Section 2 must always be completed via the 3982 label or entered manually in lieu of the 3982 label. If the customer entered their old address incorrectly and you are trying to fix it, you must enter their old address incorrectly (just as the customer did) in Section 2; then write the corrected address in Section 5. Check the Correct Old Address action box in Section 1. How do you complete the new PS Form 3546 for a customer going from address A to address B and then back to address A? This customer is requesting that the order be cancelled. Have the customer fill out a 3575 with their old address and write “Cancel” in the New Address field. The carrier or box clerk must fill out and submit PS Form 3546 with the Cancel COA; Resume Delivery action box checked in Section 1. ENTER Sometimes I don’t get a 3982 label from CFS when I submit a PS Form 3575 or PS Form 3546. A PS Form 3575 will generate a 3982 label, but a label will not be provided if a duplicate exists. A PS Form 3546 that contains a correction to the OLD address or a deletion of a record will generate a 3982 label. A label will not be generated when the change is to the NEW address.

38  A new record cannot be added. For example: No new Individual, Business or Family order can be added to an established record. ENTER  Only one record can be updated at a time per PS Form 3546. For example: One Individual only, or one Family only, or one Business only.  Start dates cannot be altered. To alter a start date a record must be cancelled via a PS Form 3546 and then resubmitted via a PS Form 3575 or PS Form 3575Z with the changed start date.  Only one record may be modified on each PS Form 3546.  Multiple corrections may be made to one record on each form. For example, the Move Type, Old Address and Temporary End Date may all be changed using one form. Check the appropriate Action Boxes and complete all applicable fields.
