5 Ensuring Safe and High Quality Standards What is Quality? Care that is clinically effective – not just in the eyes of clinicians but in the eyes of patients themselves; Care that is safe; and, Care that provides as positive an experience for patients as possible. High quality care is only achieved when all three dimensions are present. NHS England (2015)

6 Compassion in Practice The 6 ‘Cs’ strategy was launched in 2012. It centres on core values and behaviours for nursing staff to enable the delivery of high quality care and standards. Care Compassion Competence Communication Courage Commitment Nurses now have a revised Code of Professional practice - four central themes: Prioritise People, Practise Effectively, Preserve Safety and Promote Professionalism and Trust. From 2016 there will be a new process being introduced by the Nursing and Midwifery Council (NMC) for Nurse Revalidation - ensuring nurses and midwives continue to be fit to practice throughout their careers.

7 Delivery of National NHS Key Objectives Mid Staffordshire Foundation Trust made national headlines in 2009 when their poor standards of care was exposed. Following this, guidance on standards were set through the publication of national reports: Francis Inquiry – Gap analysis of those actions that were applicable for all NHS contracts, including the CCG. Berwick – Based on outcomes of Francis, offered pragmatic advice to NHS staff and patients on the implementation of the findings of Francis Keogh – Review of poor performing organisations which included putting hospitals into special measures, this included BTUH Winterbourne-Transforming care agenda, requires stronger regulation, inspection and clearer accountability NICE guidance on Safer staffing – data received gives information about staffing levels - actual versus planned ratios on wards.

8 Patient Experience The CCG will seek patient feedback in order to triangulate with other available intelligence sources to gain their perspective on the standards of care provision Incidents/complaints Friends and Family Test Quality visits Patient Safety Thermometer data which measures pressure ulcers, falls, venous thrombosis embolism and urinary tract infections.

9 Safeguarding those at Risk To ensure that policies procedures and guidelines are available for staff, to guide their practice. To ensure that staff receive the appropriate standard of training to ensure practice is in line with current legislation. Provide support for those adults and children who are at risk of, or have been abused. To undertake robust investigations when concerns are raised. To work in collaboration with statutory partners. To ensure learning is embedded from the Lampard (Savile) Inquiry. Dr Nimal-Raj

10 Quality Visits and Regulatory Inspections Both the Care Quality Commission and the CCG have a programme of quality visits. The standards that are monitored include: Person centred care - feedback from patients, relatives and staff. Dignity and respect. Consent. Safety. Safeguarding from abuse. Food and drink. Premises and equipment. Complaints. Good Governance. Staffing- including fit and proper staff. Duty of candour.

11 This year the CQC visited: BTUH in July. The hospital was given an overall rating of ‘Good’. For specific note BTUH was the first hospital to be taken out of ‘special measures’ following the Keogh review. SEPT in September and the formal report is awaited. Primary care as part of the scheduled visits programme. Reports are available from the CQC website. Two of the three Nursing Homes in the Thurrock locality; Bluebell Court was given an overall rating of ‘Good’ and Carolyne House ‘Requires Improvement’. For note there have not been any inspections to NELFT Services in the Thurrock area. We as a CCG have undertaken 39 for NELFT/SEPT/BTUH this calendar year. Quality Visits and Regulatory Inspections
