2  Student verbalizes or exhibits  Suicidal ideation or desire to harm self  Homicidal ideation or desire to harm others  Self-destructive behaviors  Non-suicidal self-injury (NSSI)

3  Staff/faculty contact the clinical counselor  Student is assessed by the clinical counselor  Assessment is based on several factors including: Seriousness of the threat Any immediate circumstances that have occurred If student is a current client

4  If student stays  Safety plan created  Appropriate staff informed  Clinical counselor contacts parents

5  If student cannot stay  Parent/guardian picks up student We stay with student until parent/guardian arrives Provide letter of understanding regarding medical leave and ability to return Provide copy of Readmission form  Verbal permission to transport We transport to public mental health clinic or ER Student is prescreened by qualified mental health provider If unsafe to transport, contact local police to assist

6  If student deemed able to return:  Qualified health care or mental health professional completes and signs Readmission form  Clinical counselor contacts parents/guardians

7  If student meets criteria for hospitalization  Local hospital/ambulance service will transport  If parent/guardian wishes to transport, stay with student until they arrive  Inform parent/guardian of Readmission form policy and provide letter of understanding via email at a later time

8  Return to school  Readmission form must be completed and signed by attending physician, mental health provider, case manager  Clinical counselor follows up with parents, student, and discharging facility  Student may be required to attend therapy with clinical counselor

9  Conclusion: This process has been successful in:  Providing guidance in a crisis situation  Enabling students to receive the services they need  Allowing a process for the student to be safely maintained  Providing a process for student to safely return school
