Ramirez et al.22 concluded from a randomized trial that intraoperative results and outcome were not affected according to whether or not the defect was sutured. The defect was left open in the majority of patients in the present study (72·0 per cent) to minimize the risk of rectal stenosis, although it was closed in patients with a high tumour.
