2. Objectives 1. Identify HIV positive persons of Puerto Rican origin who are unaware of their status or not engaged in HIV primary care 2. Engage and retain these individuals in a culturally competent, scalable, and sustainable model of integrated care 3. Provide quality HIV primary care, HIV/HCV co-infection, mental health, and substance abuse treatment 4. Successfully address contextual, cultural, and language barriers that impede engagement and retention in care 5. Evaluate this model locally and in collaboration with the national SPNS initiative

Intervention Description Priority population – Puerto Rican injection drug users Service delivery setting – Prevention Point Philadelphia (PPP) Strategies Stigma reduction – host educational workshops at PPP, conduct a traditional community assessment through PPP to identify community groups, churches, and other venues to raise awareness around HIV stigma, offer stigma reduction workshops to interested organizations Identification (outreach, recruitment, testing) - inform the community of the existence of Clinica Bienestar, identify HIV positive individuals through increased HIV testing at Prevention Point and outreach sites, place advertisement in Al Dia, the local Latino newspaper Linkage-to-Care - same day linkage of newly identified HIV positive individuals to medical case managers at Clinica Bienestar Retention - provide quality and culturally competent medical care in the community in which patients’ reside, care navigator services, transportation, culturally appropriate education initiatives, family connection initiative

We have developed our protocol and are awaiting the final national evaluation to submit it to the Philadelphia FIGHT IRB Evidence based interventions – provide collocated HIV testing and primary HIV medical care, same day linkage to medical case manager after HIV diagnosis, provide culturally competent primary HIV medical care How are you documenting the procedures you will use in your intervention – We will create a summary of findings report, a providers’ guide for HIV primary care providers, and a strategic plan for leaders of community- based health and social service agencies

8. Intervention Update What changes have you made in your intervention(s)? - We have not made any changes to our intervention. What challenges are you facing in design/implementation of your intervention? We have not faced any challenges in the design/implementation of the intervention. What is the timeline for implementation of your intervention(s)? – We started piloting our intervention in December 2013. We are prepared to scale up our HIV testing and outreach to community partners after July 1 st.
