7 The QAF scores D - The provider does not identify and record the social and spiritual needs of the service users. C- The provider identifies and records service users’ social and spiritual needs. A programme of activities is on offer, but does not reflect the recorded hobbies and interests of the service users. B- An activities programme is available and created to reflect the hobbies and interests of the service users. Specific needs such as disability, hearing loss or dementia are taken into consideration when creating the programme. A- Records are maintained to reflect each person’s participation in activities. The activities programme is evaluated regularly, and amended to reflect changing needs.
