1 HOW TO USE THIS TEMPLATE CHANGING THE LAYOUT This template has several different column layouts. Right-click your mouse on the template background and click on “Layout” to see different layout options. The column widths in these preformatted layouts cannot be moved but advanced users can modify any layout by clicking on the VIEW menu and then on SLIDE MASTER. CHANGING THE COLOR SCHEME To change the color scheme of this template click on the DESIGN menu and then on COLORS. You can choose from the provided color combinations or you can create your own. USING PLACEHOLDERS To add text to this template, click inside a placeholder and type in or paste your text. To move a placeholder, click on it once to select it, then place your cursor on its frame and then click and hold as you drag it to its new location. Resize the placeholder, if necessary. Placeholders for headers, text and graphics can be found below: HEADER PLACEHOLDER Move this preformatted header placeholder to the poster area to add another header. Use headers to separate topics or concepts within your presentation. TEXT PLACEHOLDER Move below text placeholder onto your poster to add a new text box. Catholic Charities Medical clinics prioritize preventive care as part of a comprehensive and patient-centered model of care. We partnered with the DC Department of Health and the DC Primary Care Association to offer the Chronic Disease Self-Management Program (CDSMP) to our patients. Chronic Disease Self-Management Program was developed by a team of researchers at Stanford University. It’s a self-management education workshop for people with a variety of chronic health conditions. It aims to build participants’ confidence in managing their health and keep them active and engaged in their lives. The Spanish version is called “Tomando Control de su Salud”. Purpose Improve physical and psychosocial outcomes and quality of life for Latino immigrants living with hypertension and other chronic conditions through the Chronic Self-Management Program. Outcomes Patients participating in Tomando Control de su Salud will show a reduction in at least one of the following measures: blood pressure, weight, BMI, abdominal circumference, LDL or HgA1c. At the end of the workshop, patients will be able to name two specific behavior changes that have resulted from participating in the course and one behavior that they will maintain after the end of the program. INTRODUCTION Our DC Medical clinic offered two six week workshops conducted by two staff members certified in the Stanford method’s CDSMP. Our Business Data Analyst pulled out from eCW a list of patients diagnosed with hypertension that attended the medical clinic during the months of January-June 2015. A total of 102 patients were identified and called to start inviting them to the workshops. Participants attended a 2 ½-hour interactive workshop once a week for six weeks to learn problem-solving, decision making, and other techniques for managing problems common to people with chronic diseases. At each class, participants set a realistic goal for the upcoming week and developed an action plan for meeting that goal. They reported on their progress at the following class, and solicited feedback from the group to help address any challenges. Key activities included interactive education, discussion, brain storming, and practice of action-planning, behavior modeling, problem-solving techniques and decision making. Symptom management included exercise, relaxation, communication, healthy eating, and medication management. At each session, a medical assistant took patients' biometrics such as BMI, abdominal circumference, and BP. LDL and HgA1c levels were extracted from patients' electronic records. At the first and sixth class, patients completed a knowledge questionnaire. 20 patients diagnosed with hypertension attended the CDSMP workshops with more women attending than men. Only 55% completed elementary school. For Abdominal Circumference (AC), a change of more than one inch was considered significant. The reduction in AC was 2.2 points, which was great to accomplish in such a short period of time. For LDL cholesterol, a change of 5 mg/dL was considered significant. Although there was a reduction in LDL of 4.31 it was not significant enough. Even though all of the patients were diagnosed with hypertension, the average BP reading was normal pre and post intervention (Table 1). About 90% of patients were taking anti-hypertension medications. Patients’ knowledge on hypertension improved after the 6 sessions (Table 2). It is important to mention patients learned how to measure food portions and make action plans to improve their chronic condition by the completion of the workshops (Table 3). Evidenced-based, self-management education programs have been proven to significantly empower people with chronic diseases. Tomando Control de su Salud participants gained an increased understanding of how to live healthier lives through taking their medications, making better food choices and engaging in physical activities. We need to continue these workshops to help support patients’ positive behavior changes. We plan to invite successful patients to lead some of the discussions and continue to implement more workshops throughout the year using additional local funding sources. The Latino community in Washington DC experiences an overburden of obesity, diabetes and chronic disease, along with poverty. By educating and empowering this population with healthy lifestyle tools and resources, they will be able to access and choose healthier options to make healthy living sustainable for them and their families.
