1 Services for the Youth and Perinatal team (Specifically the 0-4 age group) Direction programme jeunesse Centre Intégré de Santé et Services Sociaux de Laval (CISSSL)

2 What is the Centre Intégré de Santé et de Services Sociaux de Laval ? Exists since April 1st 2015 following the application of Bill 10. Merger of the CSSS de Laval, Centre jeunesse de Laval, CRDITED de Laval, l’Hôpital juif de réadaptation and the Direction de la santé publique de Laval.

3 The Youth service (Programme Jeunesse) at the CISSS Laval Youth teams from the former structure of the CSSS Laval: – Application of mesures and placement of the Director of Youth Protection. – Former Family-Children and Youth team ( Famille-Enfance-Jeunesse) from the CSSS Our coordination: 0-12 ans – Perinatal team (specific age group) et 0-4 ans – Youth with Difficulties Team 5-12 years + Speech and Language Therapy and development clinics – Application of measures team 0-4 years – Application of measures team 5-12 years

4 Perinatal team (specifially age group) and 0-4 years old programme OLO (Eggs, Milk, Orange) programme SIPPE (Perinatal and Young children) programme JED 0-4 ans (Youth in Difficulty)

5 1) OLO Program Eggs-Milk-Organge (OLO-Œufs – lait – oranges) Targeted for pregnant women, 12 weeks or more – Who are below the low-income cut-off – Who has completed at least one diploma (or the equivalent or more of 11 years of study in their country)

6 OLO Program Nutritionists are ‘Case managers’ First meetings take place at home Perinatal groups are held in community organizations (CCVM et Entraide Pont-Viau), Co- faciliatated by a nutritionist and a nurse. OLO coupons and vitamins are handed out A post-natal group meets

7 Expansion of the ‘OLO’ approach Projet 1000 jours pour savourer la vie (1000 days to savour life project) – Aims to promote the adoption of healthy food habits in the first 1000 days of life of the child who is living in a situation of vulnerability (270 + 365 + 365). – Production of tools such as recipe book with imgages, meal and purchase planners, livres pour books for babies, etc.

9 2) SIPPE Program Integrated perinatal and services for young children Target for pregnant women,12 weeks or more or who has a child under 1 year in their family – Who lives under the low-income cut-off AND has not completed secondary V (High school) (or has not completed 11 years of study in their country) OR – Has immigrated to Quebec in the last year AND living under the low-income cut-off

10 SIPPE Program Prevention and health promotion program Objectives: – Promote the development of children aged 0-5 years – Maximize the health and well-being potential of families. – Improve the lives and condition of families.

11 SIPPE Program Services… … rest on early intervention, that is intensive, continuous, and that is adapted to the situation and needs of vulnerable families Multidisciplinary team: – Nurses – Psychosocial professionals (psychoeducators et Social Workers) – Nutritionists – Speech and Language therapists – Occupational Therapists – Psychologists – Homecare workers (Auxiliaires en santé et services sociaux)

12 SIPPE Program The nurses are case managers in files – First contact with a nutritionist however – OLO coupons and perinatal vitamins – Personalized pre-natal coures offered in the home – Multiple steps (community) and referrals (to other professionals internally or outside) Psychosocial professional – Case manager when a family or the mother presents with serious psychosocial problems which has an impact on her child.

14 3) JED program 0-4 years old Youth in difficulty 0-4 years (Jeunes en Difficultés) Concerns all familiies with at least 1 child aged 0 to 4 years having: – Indicators of developmental delays – Behavioral problems – Relational difficulties between parents and child – Difficulties adapting to social situations – Issues with managing routines and parental tasks – Etc…

15 JED program 0-4 years old The Psychosocial professional is case-manager in file Span of service is approximately 15 meetings – 2 to 3 meetings for evaluation – 12 meetings for follow-up using the guidance of an intervention plan that was developed conjointly with the family Support for parental skills Support for behavior and adaptive strategies for children Stimulation activities (Pistes de stimulation) Screening Referrals – Speech and Language therapy clinic ( ~ 12 month wait) – Developmental clinic ( ~ 18 month wait) – Other professionals or group activities within our team Multiple tasks

16 JED program 0-4 years old *Note: As of September 29th 2014, all children who are referred to the development clinic by a partner (ex: doctor, day care, etc.) must withouth exception have completed the JED program in order to: – Confirm the routing of the file to the developmental clinic – Support the parents in their day to day lives – Offer avenues for stimulation and strategies to help in the development of the child while waiting for their evaluation

18 Issues often experienced by our families Sever financial difficulties Difficulties adapting to their role as parents Lack of knowledge regarding the needs, the development or the security of their child/children Problems with addictions and or mental health problems Difficulty with the management of the household Situations of negligence, abuse and/or violence Context of seperation and or mourning.

19 Side note: Programme PIILE Integrated intervention program of Laval for Children (Programme d’interventions intégrées lavallois pour les enfants) Targets families with at least one child aged 0-12 years (Complementary program) Some of its objectives: – Develop parental skills and reduce stress – Avoid reporting to the Director of Youth Protection – Reduce the use of placement – Promote the social integration of families

20 Side note: Programme PIILE 3 levels of intervention: – Developmental needs of the children – Capacity of parents to answer these needs – Conditions in the social and family environnement – Partners: – Youth teams within the CISSS – Community Organizations – Day care centres – Schools and school community – Etc.

21 Side note: Programme PIILE Eligibility: – A family cannot be referred by an outside organization. – Must have followed a regular service but has not answered the needs of the family. – Interventions should have spanned 2 years with a high level of intensity.
