Sauk County, Wisconsin December 2010 Bus was traveling 16 miles per hour over speed limit on icy roads Operator lost traction control and over-corrected causing the bus to roll. 7 of the 11 students on board were transported to the hospital. Operator was not injured, but lost his job. Damage to bus estimated at $10,000.

February 2011 Semi side-swiped one bus, hitting another head on. 62 students on board the 2 buses (which were on a field trip) 2 school employees, one of whom was the bus operator, and the semi driver were killed. 10 students transported to the hospital, 1 by helicopter. Because both drivers died, no “final” cause for the collisions has been determined.

4 www.FLDOE.org © 2014, Florida Department of Education. All Rights Reserved. Ohio October 2010 Bus operator was distracted by students. Bus left the roadway onto embankment. Operator tried to correct the bus, but it rolled. Several students sustained minor injuries.

5 www.FLDOE.org © 2014, Florida Department of Education. All Rights Reserved. Ohio November 2010 Bus operator was distracted by incident inside the bus. Went off the roadway and tried to get back on, but the bus tipped to the right. Bus came to rest against structure.

A Semi (no trailer) had slowed for construction and was rear ended by a pickup truck. The operator of the first bus did not see the collision and hit the pickup truck, causing it to go up on the back of the semi. The second bus hit the first bus so hard, that the impact pushed the first bus up onto the pickup and through the roof of the tractor.

She was sitting in the rear right side passenger seat on the first bus. The driver of the pickup truck was also killed. NTSB determined that the driver texted 11 times in the 11 minutes before impact. 50 students were injured. Only 8 students were transported to area hospitals. Speed limit on I-44 was 70MPH. It was suspected that the buses may have been speeding.

(con’t) August 2010 NTSB determined that the ABS indicators on both buses were not operational, and that ABS on the first bus was not functioning at the time of impact. Back-up hydraulic brakes kicked in. The second bus has malfunctioning ABS on the right rear tire. Also, an existing brake line leak indicated fluid leaking when the brake was depressed, and the bus company had fabricated a piece to the brake system. The bus company could not produce maintenance records for either bus and there was no routine inspection in place.

Bus Operator swerved at last minute to avoid a more serious collision. 45 students on board, seven and the bus operator taken to the hospital with minor injuries and released. Driver of third vehicle was killed on scene and her 7 year old daughter sustained minor injuries.

Volusia County March 2015 School Bus was stopped with students lights activated. 3 students on board the bus (none injured). 2 children in the car (injuries unknown) Driver of the car was ticketed with Careless Driving and Driving Without A Valid License.
