2. Please pull up the SLO template. All teachers will pilot ONE SLO this year. We will be using MAP and PMAP scores “Interval of Instruction” will be Fall MAP to Spring MAP (other SLO’s could be different) DO will pre-fill several of the boxes. SLO’s for 2015- 16 It will NOT be part of your 2015-16 evaluation

3. Please type in your name, school, and the content area that you are using for your SLO. What group of students will you be including in your SLO? ELA or Math? Which specific standards will your SLO target? DO will pre-fill several of the boxes.

4. This has already been filled in for you for this year. These percentages are different from previous ratings. We will use the MAP Student Progress Reports to complete this table. You should use ALL of your students or a minimum of 10 since this is a pilot.
