3 Arizona Early Intervention Program (AzEIP) $2,920,000. Statewide AzEIP provides early intervention services to families with children birth to 3 who have developmental delays and/or disabilities. Early intervention provides support to families and caregivers within a Team Based coaching model through everyday learning opportunities and in the child’s natural environment. Phoenix 19 staff 6 subcontractors Pima 19 staff 7 subcontractors Yuma 8 staff 4 subcontractors 46 staff and 17 subcontractors

4 Building Bright Futures $1,416,000. 21 staff statewide (16 in Phoenix) Parents as Teachers: A different flavor of Home Visitation *Evidence Based *Affiliate Status with National Office *Target Populations Individual Home Visits Screening/Monitoring of Child Development and Progress Group Connections Networking and Coordination of Services

6 Smart Support Mental Health Consultation $290,565 4 staff statewide (3 in Phoenix, I in Yuma) Smart Support provides mental health consultation services to early care and education providers to promote the social and emotional development of all children in their care and help them respond to children with behavioral challenges. CFR is a subcontractor for Southwest Human Development who holds the statewide contract with First Things First.

7 Maricopa Center for Adolescent Parents $326,000 5.5 staff in Phoenix + 3 MSW Interns Annie E. Casey Foundation recently issued report on benefits of a two-generational approach to services Operational for 20 years in the Phoenix office Comprehensive set of services for teenage mothers and their children including a licensed, nationally accredited, 4 star rated Early Learning Center Only program without at designated funding source other than child care scholarships and the Helena Harvey Endowment, which makes up approximately 50% of the needed annual funding.

10 Teen Parent Support Program $306,000 5 staff in Phoenix Hybrid model including home visitation and community based parenting education. Evidence based utilizing Nurturing Parenting curricula and outcome measurement tools. Case management services Group parenting classes Developmental Screening Information and Referral Child Care Scholarships solely for teen parents

11 Bright Choices Parenting Education $187,000 3 staff in Phoenix Community Based Parenting Education program utilizing evidence based Nurturing Parenting Curriculum Collaboration between Arizona’s Children Association and CFR with CFR as the subcontractor
