3 Houston Top 10 PSC Tank Vessel Deficiencies 2008 – Present 77- Pollution Prevention/MARPOL Equipment (Pollution/Response) 46- Hold/Tanks (Decks/Cargo) 40- IGS (Decks/Cargo) 37- Cargo Transfer/Lightering (Decks/Cargo) 31- Certificates/Documents (Documents) 25- Lifeboat Equipment (Lifesaving) 21- Certificates/Documents/Licenses (Personnel) 20- Diesel Engine (Engineering) 19- Vapor Control System (Deck/Cargo) 17- Lifeboat (Lifesaving) The above deficiencies make up approximately 54% of all deficiencies issued to Tank Vessels examined in Houston since 2008. Total Defs issued: 616 Top 10 account for 333

5 Lifeboat Casualty – Houston Ship Channel – August 2010 This visible bump on newer style hooks indicates the hook is not locked. On-Load Release System

6 Lifeboat Casualty – Houston Ship Channel – August 2010 Fresh caramel color where grease was applied with a brush. Same hook was cleaned, lowered & lifted from greasy brackets. On-Load Release System

7 Lifeboat Casualty – Houston Ship Channel – August 2010 Davit design allows for lifeboat to rest on a Keel Rest when suspended from davit. On-Load Release System

8 Non-Class Hose The hose ran from the Low Temperature Cooling Tank to the top of the MSD. The T fitting is shown in the picture connected. To put the T in the crew would have had to cut the air vent lines that keep continues pressure between the two tanks in the MSD. MSD Pollution Case – OCT 10

9 Low Temp Cooling Tank Tracing the hose led to the LT Cooling Tank. You can see the vent for the tank on top. The rubber hose runs several flats down to the MSD. Normally LT Cooling Tank overflow would flow down to the Bilge Holding Tank. The LT Cooling Tank services the jacket water cooling. MSD Pollution Case
