1 MA1603 – Clean Harbors Environmental Service MA1604 – Veolia ES Technical Solutions

2 Hazardous Waste Contractors provide all labor, materials, equipment, paperwork and services Contractors follow EPA, DOT, OSHA, and State of Utah regulations. Upon notification, contractor has 30 days to remove waste. Emergency removal within 48 hours.

3 Hazardous Waste Contractor takes ownership of waste. Contractor will provide documentation showing: EPA ID # Copy of Certificate Contractors liability insurance.

4 Hazardous Waste If you have any questions regarding the hazardous waste contracts you can view them at: Purchasing.utah.gov, or Contact Ann Schliep @ aschliep@utah.gov
