2  A form of assessment in which students are asked to perform real-world tasks that demonstrate meaningful application of essential knowledge and skills -- Jon Mueller

3  Authentic assessment are assessments such as portfolios, projects, labs, essays and rubrics, where students have to perform meaningful tasks, demonstrate proficiency, and apply their knowledge to real- world situations.  Traditional assessments are assessments which require students to select an answer or recall information such as multiple-choice, fill-in- the-blank and true/false assessments. http://jfmueller.faculty.noctrl.edu/toolbox/whatisit.htm

4  Like Authentic Assessments in the classroom, the goal of transition assessments is to find out new, meaningful information about your student to help guide their postsecondary and annual goals.

5  For transition age students we are required to put transition services/activities in the IEP to show what we are doing to help the student reach their postsecondary goals.

6  You can use the information you gathered from the activity as an authentic transition assessment to use for the next year’s IEP and the student can be an active participant in the process!

19 Find out what your students are doing in the general education classes (ask their teachers) Here is an example of a form you can use to find out what is being done in the gen ed classes already. General Ed Teacher Survey

20 Find out what your student is doing in the community

21  If you can’t find an assessment that fits the situation..create your own!  You do not have to be the one to do all the transition services/activities with the students.  Choose MEANINGFUL assessments that provide USEFUL information. Otherwise it IS just paperwork!
