The Nominees (L to R ) Dr Kathy Savoia – Physician, Barb Strother - Clinic Manager/Panel Coordinator Nominated by: Julia Vanden Heuvel & Terri Gilson (Improvement Facilitators, Calgary Foothills PCN) Why Nominated: New category – spirit award! 2 woman team; Dr. Savoia describes Barb as the yin to her yang; Barb has played a key role in the panel clean up – super organized Together they took on Alberta Screening and Prevention - they quickly realized that panel clean-up was important and it wasn’t as overwhelming as it appeared in the beginning (this is one of their key messages they share at the Physician Led Workshops for PMH and Panel - sponsored by CFPCN) Dr. Savoia really reaped the benefits of a clean panel when she wanted to do a mental health screening project this year to more easily identify her patients in need of mental health screening - It was a much smoother process with a clean panel! Like their colleagues Dr. Bland/Nicole (Family Health Clinic - Calgary Foothills PCN), Dr Savoia/Barb have led by example and have inspired other physicians and clinic teams to engage in the work of panel For all her hard work Dr. Savoia took Barb to Las Vegas! The Nominees (L to R ) Nicole McAuley - Clinic Manager/ Panel Coordinator, Dr. Elaine Bland – Physician Nominated by: Julia Vanden Heuvel & Terri Gilson (Improvement Facilitators, Calgary Foothills PCN) Why Nominated: In a 10 physician clinic, Dr. Bland was willing to be the “guinea pig” for the work of panel and Alberta Screening and Prevention (ASaP) - she’s not only a leader for this work in her clinic, along with Nicole she’s also inspired other CFPCN physicians to reap the benefits of an identified and clean panel (via the PCN sponsored Physician Led Workshops for PMH and Panel) Cleaning up her panel was a team effort (back to the front) Her fellow colleagues (at least 4) agreed to do panel work and to join ASaP based on her example Having a clean panel has created a strong foundation to engage in other initiatives to improve the way she and her team deliver patient care (Choosing Wisely is next) Behind the scenes – Nicole has played a key role in driving this work forward. The Nominees (L to R ) Carla Mazur – RN, Dr.Lori Gohill, Skende Huskic - Office Manager/Panel Manager, Dr.Harjot Singh, Dr.Christina Siauw, Dzemila Hasific, MOA, Lisa Gooliaff, MOA (missing Dr. Cassandra Millar) Nominated by: Skende Huskic – Office/Panel Manager Why Nominated: Skende shares the work of panel (identification, maintenance and management) has been a team effort at her clinic – front to Dr. Singh - everyone engaged b/c they could see the value it would add to patient care – they were all willing to invest the time. Skende is very proud of the team’s willingness to participate in the work and above all the fact that it’s improved the quality of their patient care (patients don’t get missed) In conjunction with their EMR provider (Med Access) the team invested into harnessing the power of their EMR – starting with complex care plans. Dr. Singh herself invested non clinic time to clean up each patient profile for complex care plans. The team worked on a plan for 1 year recalls of CCPs Dr. Singh had done great work in cleaning her profiles, seeing patients for CCPs – she presented her work at a Conference in Banff (last year) The work of panel has opened the door for the team to work on sub panels (mammogram recalls, pap tests, prostate screening) Next steps: standardization amongst the physician panels (data input) Drs. The Nominees (L to R ) Dr. Simon Warwick – Physician, Shayla MacDonald - LPN-Nursing Lead, Dr. Carly Crewe – Physician, Leah Hayes - LPN-Physician Assistant, Dr. Jena Smith – Physician, Jen McKinstry - Administrative Assistant , Dr. Rob Warren – Physician, Stacey Johnson - Clinic Manager, Cheryl Lindberg - Administrative Assistant, Dr. Michelle Warren - Physician , Leah Jensen - Panel Coordinator/ Improvement Facilitator Nominated by: Dr. Robert Warren & Dr. Michelle Warren (Clinic Owners)  Why Nominated: Clinic was founded in from the initial planning stages the vision of the clinic was to create a model of primary care that was highly patient centred using a team-based multidisciplinary approach Doctors Rob and Michelle Warren have empowered this team to create and modify the processes they currently follow. "There is no perfect system so do not fear change" All members of the team participate in QI, we proceed from "plan" to "do" so quickly that often new ideas are preceded by the statement "I'm not saying we need to do this right away, but …“ Leah Jensen, Stacey Johnson, Shayla MacDonald They identify issues early and come to the physicians with both the issues and proposed solutions They have become facilitators for Alberta Screening and Prevention and AIM and have served as a resource to other clinics both within our PCN and other communities that are just beginning the process They have worked with our EMR vendor, Telin systems, in detail to publish training videos on panel management, the periodic office encounter manager (POEM) and use for ASaP maneuvers They each bring strengths to the team and work in an amazing collaborative manner that has set the clinic up as being an exemplary medical home for their patients “This is a highly motivated team that obviously takes pride in the service we provide to our community and the reputation we have built in a very short time. We are fortunate to be surrounded by a group of people who 'take ownership' in the service and take pride in seeing patients receive the highest level of care. The Nominees (L to R ) Janyce Tookey – Improvement (Panel) Coordinator, Angie Weston - Assistant Manager, Lana deBoon - PRPCN Executive Director * Input from all staff Nominated by: Lana deBoon Executive Director, Peace River PCN Why Nominated: The physicians at Peace River Associate Medical Clinic put their trust in the team, led by the PRPCN Executive Director, in taking the first steps towards Panel Management. Core team received Panel Management and Improvement Facilitator training This team cleaned up their panel up (from 66,000 to 10,000 patients), Lana estimates she spent a total of 6 hours (including meetings with the improvement team)... Core team requested input from everyone at weekly staff meetings. Lana reflects – “I believe this is why the implementation went so smoothly, as all staff were involved from the beginning and they had time to ask questions and understand why panel management was important for our patients” We have a very small, rural team and yet we feel like we have made a difference for our physicians, staff and especially our patients Associate Medical Clinic Peace River PCN

The team has my support to ensure these goals are met The Improvement Team meets weekly to discuss initiatives, areas that are going well and areas for improvement The Improvement Team and other PCN Team members are all very motivated toward making positive changes and ensuring the PCN patient panel is well identified as well as ensuring we provide consistent quality patient-centred care to improve clinical outcomes Cold Lake PCN

Nominated by: Nataliia Slobodianyk (PCN Evolution Project Manager, SCPCN) Why Nominated: In close partnership with South Calgary PCN these 3 clinic teams were willing to lead the way for others to follow and engage in the work of building PMH, starting with panel After sharing their experiences at a PCNe Forum in September, 9 new clinic teams (16 physicians) agreed to engage in the work of panel As teams each clinic has: completed panel identification and validation used patient feedback from patient satisfaction survey to improve patient care done workflow assessment and management participated in health team effectiveness activities engaged in chronic disease management (EQ-5D-5L) monitored access (TNA) evolved proactive care - opportunistic screening They all attribute the success they’ve experienced thus far largely to a team effort.
