5 1. Reduce Symptoms Percentage of persons who demonstrated a reduction of mental health related symptoms, such as traumatic stress, medical/physical, substance use, anxiety, depression, hyper affect, and disturbed thought process or cognitive performance from admission to discharge, or every six months. Scales: Depression + Anxiety+ Hyperactivity + Thought Content+ Substance Use Success  Current score < Prior score

6 2. Improve Life Skills Percent of persons served by the Department’s SAMH funds that improve their ability to care for themselves and manage their activities of daily living from admission to discharge. Scales: ADL Functioning + Self Care (Adults only) Success  Current score < Prior score

Increase Positive Behaviors Percent of persons served by the Department’s SAMH funds that show improvement in their interpersonal and family relationships, family environment or behavior in their home setting, and in their work or school environments from admission to discharge. Scales: Interpersonal relationships + Family relationships (Adults only) + Family Environment (Adults only) + Behavior in “Home” Setting (Children only) + Socio-Legal + Work/School Success  Current score < Prior score

8 4. Satisfaction Surveys Percent of persons served by the Department’s SAMH funds that are satisfied with the services they receive. Results based on number of surveys submitted and number of answers that were positive.

Increase Safety Skills Percent of persons served by the Department’s SAMH funds will show a reduction in criminal/juvenile justice involvement, harmful behavior to self or others, and security management needs. Scales: Danger to Self + Danger to Others + Security Needs Success  Current score < Prior score

10 In all categories the number of FARS/CFARS, both prior and currents are extremely important because the calculation of the percentage is done as follows: Prior assessment score = A Current assessment score = B Successful outcome = C Unsuccessful outcome = D If (B A), then = D Determine ratio of successful outcomes o X = Successful outcome ratio o X = C/(C + D)
