20 What You Need to Know For Smooth Transition – Part C Conference n Who must attend? –Parents –Family Service Coordinator –LEA representative for Part B 619 services –Representative and/or provider(s) of the services the child may be eligible –Any other person or service provider(s) invited by the parent or the LEA who might help support and develop the transition plan

21 What You Need to Know For Smooth Transition – Part C Conference n What if Part B 619 staff are unable to attend? n Part B is required to attend – but if they can’t this cannot be the cause of a late conference n The local tiny-k program is still responsible for convening a timely transition conference n The local tiny-k program conveys the information required by the Part B program to the parent

22 What You Need to Know For Smooth Transition – Part B n Must participate in the transition planning conference arranged by the Part C program n For a child served under Part C, at the request of the parent, invite the Part C Service Coordinator or other representative n Have developed and implemented by the child’s 3 rd birthday an IEP or IFSP

23 What You Need to Know For Smooth Transition – Part B n I f an IFSP is used for a child age 3 to 5 –IFSP must include natural environments statement –Implement requirements for pre-literacy, language and numeracy –Developed in accordance with IEP procedures and timeline –Provide the parents an explanation of difference between an IFSP and IEP –Obtain written consent of parent for use of IFSP
