It contains:  how I am doing in school  my goals for the year  I can:  help set my IEP goals by expressing what I think is important to me  help plan the assistance I need to reach these goals  participate in my own IEP meeting

7. reatment plan with my PT “PRACTICE, PRACTICE, PRACTICE!”  In order to reach my goals, I need to work with my PT to figure out:  What exercises and activities do I need?  What equipment do I need? (wheelchair, crutches, leg brace, special seating, etc.)  What accommodations do I need? (more time to get from one classroom to another, stand in front of line, etc.)  What assistance do I need? (supervision, physical assistance, etc.)

8. dvocate for myself “EVERYBODY NEEDS HELP SOMETIMES.”  When I am unable to do something by myself, I can ask others for help.  For example, I can ask others for:  directions, detailed instructions  physical assistance  special accommodations  I will learn to communicate how I can be assisted.

9. earn from my actions “EVERY TIME I TRY TO DO SOMETHING, I CAN LEARN SOMETHING NEW!”  I can learn to monitor my own progress.  I can learn to tell whether something is working or not.  If something is not working, I should change my approach or try something else that might work.  If something is working, I should remember how I did it correctly.  I can learn to control my behavior and emotions in different situations.
