ANSC 7035 (10-04) SECTION IV – The DSO-PS verifies that all the information is correct, checks off the eligibility for Retired Status box, signs, dates, makes a copy of request and emails to DIRAUX. Notice is sent to the DCDR informing him of request. x SECTION II - Member checks “I desire to Retired Member status” box, enters date of enrollment, signs and emails to the FC x SECTION III - FC checks Retired Status Box,, verifies that all the information is correct, dates, electronically signs, and emails to the DSO-PS. x Date x DSO-PS Signature Date RETIREMENT REQUEST. Date FC Signature Flotilla # Enrollment Date Member Signature SECTION I – FC enters Member’s name and ID Number for all requests.

ANSC 7056 (10-04) MEMBER TRANSFER REQUEST ANSC 7056 SECTION 1 – CURRENT INFORMATION Member inserts name, member number, current flotilla number, Flotilla Commander’s name, effective date and desired flotilla. Member signs, dates and emails both forms (ANSC 7035 and ANSC 7056) to his FC. Current Flotilla Commander verifies that the Member is in good standing, with all dues paid, any flotilla property returned and recommends approval. If not approved, reasons must be submitted in writing. FC SIGNS, DATES AND emails 7035 AND 7056 TO THE DSO-PS If personal changes are involved such as change of address, the ANSC 7028 should also be submitted.. x Member Signature Date. ANSC 7035 (10-04) DECEASED MEMBER SECTION III – Flotilla Commander checks the “Death of Member” box, enters name of deceased member, name and address of next of kin (including their relationship to the Member and date of death per District), signs, dates and emails to the DSO-PS AT THE TIME OF DEATH, FC SHOULD SEND PHOTO OF DECEASED MEMBER TO THE DSO-CS. x FC Signature Date

ANSC 7035 (10-04). x FC SignatureDate MEMBER DISENROLLMENT REQUEST SECTION II – Member fill in the Flotilla Number, dates and checks “I desire to disenroll” box, writes a short reason for disenrolling, signs, attaches membership card and emails to the FC SECTION III – FC checks “Member Request” box, verifies that all the information is correct, signs, dates and emails to the DSO-PS. If the ID Card is included, FC must mail the packet to the DSO-PS. x Date Member Signature

8 7Copyright ©2004 Coast Guard Auxiliary Association, Inc. ANSC 7035 (10-04)... Each Flotilla sends out dues notice with a due date for payment, Members must be given thirty (30) days to respond. If the member has not responded within 30 days, a second notice with the 7035 should be sent explaining that no response was received. The member is directed to complete the 7035. PER THE FLOTILLA PROCEDURES MANUAL, THE FC SHOULD MAKE EVERY EFFORT TO CONTACT AND RETAIN THE MEMBER. SECTION II – member indicates whether he wishes to remain in the Auxiliary, if so, signs and sends in his dues. SECTION II – If member decides to be disenrolled, the ”I desire to disenroll” box should be checked, the reason for disenrollment, signs where indicated and returns his membership card. SECTION I – FC fills in necessary information, signs and dates form. The 7035 along with the second notice is mailed to the member via CERTIFIED MAIL with RETURN RECEIPT REQUESTED. SECTION III - If no response is received after 30 days, the FC indicates disenrollment, signs, dates and submits 7035 along with the CERTIFIED MAILING RECEIPT REQUESTED to the Division Commander who signs, dates and mails to the DSO-PS x Date x Member Signature x x XXXX X XXX X X X xxxx x FC SignatureDate FC Signature DCP SignatureDate x x DISENROLLMENT FOR NON-PAYMENT OF DUES
