Completing the Membership Data Form (MDF) It is important that the information is accurate since that is how the employee’s account will be set up in our member system.

Completing the Membership Data Form (MDF) Employee Completes Part 1 Employee Completes Part 1 Part 1 of the form is for the employee’s personal data.

Completing the Membership Data Form (MDF) Employer Completes Part 2 Employer Completes Part 2 Check over the form carefully to determine prior coverage. Ask the employee if s/he has ever worked for the state, county, city or for a school in a non-teaching position. All state agencies and county offices in Arkansas are covered by APERS. Contact the APERS Call Center if you are unsure whether a city is a participating employer.

Completing the Membership Data Form (MDF) Social Security Card Social Security Card A copy of the employee’s social security card must be submitted with each MDF.
