35 the quoting process Two things are needed to request a GSI quote: Completed Request for Proposal (RFP) form Include a complete employer electronic census (Excel file) of all employees including: Name Date of birth Gender Home address Job title and duties, if possible Annualized base income Bonus or commission – 2-year history (not the average) needed for variable income Details of any group LTD or other individual DI coverage in force LTD booklet preferred DI1579 12-14For producer use only.
