2 Ben Knight C&WCVDNw 21.9.2012 The Project - Aims To help those affected by a stroke to improve their health and wellbeing by using coping and mindfulness strategies to gain control of their situation to enable a return to as normal lifestyle following a stroke with tailored strategies to boost their confidence and self esteem and thereby enhance their ability to better manage their condition. To skill key members of the workforce in the Thistle Self- Management methodology to enable further courses to be run locally and embed this into the recovery pathway reducing the need for high level psychology services and reduce the risk of readmission.

3 Ben Knight C&WCVDNw 21.9.2012 Drivers To achieve the agenda of The National Stroke Strategy (DH, 2007) and Accelerated Stroke Improvement (ASI) Programme (DH, 2010) to improve post hospital and long term care and to increase access to psychological support in line with commissioning intentions To follow the QIPP for self-care support for long term conditions produced by the Department of Health (2010) in reducing dependency on NHS To promote recovery, and to improve quality of life, reduce and prevent depression and anxiety (Stoke Strategy Quality Marker 13) RCP National Clinical Guideline for Stroke Recommendation (Fourth edition) 6.34.1 Step 2
