8 Project Narrative & Review Criteria 8 Project Narrative provides a comprehensive description of all aspects of the proposed project Review Criteria are utilized by reviewers to assess the application: Need (10 points) Project Update (15 points) Response (35 points) Resources/Capabilities (10 points) Evaluative Measures (10 points) Impact (10 points) Support Requested (10 points)

9 Performance Measures Applicants must include at least 6 of the 14 sample core Clinical Performance Measures 6 additional sample areas for inclusion of additional performance measure goals: –Behavioral Health –Oral Health –Hearing/Otitis Media –Traditional Healing –Health Education and Disease Prevention –Nutrition and Physical Activity Applicants must include at least 2 of the 6 sample core Financial Performance Measures 9
