4 Conclusions of Investigation  Mine operator had not provided newly employed, experienced miner training or task training  Contractor had not provided newly employed, experienced miner training or task training (Overlapping citations issued)  Newly employed, experienced miner training document (5000-23) indicated that training had been provided by an uncertified instructor  Proper equipment blocking techniques were not used  Coal truck driver was performing duties of a crane mechanic and was obviously inexperienced at task being performed
