Time, Talent, Treasure & Tomorrow Instructions for Part CD QI Initiative Face-to-Face Session Storyboard: Time, Talent, Treasure & Tomorrow Each Storyboard should consist of 5 slides: 1 cover slide that identifies your agency, 4 additional slides that describe an improvement project implemented in 2012 or your planned 2013 project What project did you select and why/how was it selected? or what project do you plan on implementing and why? What improvement methodology was used and what tests of change did you employ? or what do you plan to do? What measures did you use to track your progress? or what measures will you use? What were your results? or how will you know when it’s time to systematize change (what is your goal)? Please feel free to personalize your slides! Add color, unique fonts, pictures, graphs, designs….anything that helps to tell your story!

2. Agency Name Consider including the following: Where are you located? How many patients do you see? What is your mission? Pictures, if possible!

3. Your QI Project Consider including the following: What are you seeking to achieve? What is your goal? Reasons for selecting this project: What does this mean for your agency? for your staff? for your patients? is it important to them? How does this fit in with National HIV/AIDS Strategy, National Quality Strategy, and other national initiatives?

4. Your Methods and Tests Consider including the following: Tell us the model(s) used Plan-Do-Study-Act, lean, other rapid vs long cycles, rule of 1 vs all patients What tests of change were employed? You can consider adding an extra slide or two here to talk about several tests of change if you tried several things Gantt charts to show implementation over time What lessons were learned along the way through these tests? Anecdotes or quotes

5. Your Measures Consider including the following: definitions of your measures relevance or evidence from literature as to why these are good choices data in its many forms (tables, run charts, control charts, sparklines, etc)

6. Your Results Consider including the following: What did it take for you to feel that you identified the change to systematize? How are you ensuring sustained improvement? What overall lessons did you learn from this in terms of QI project management? Anecdotes or quotes
