3 + Growth Chart Height: 23.5in Height Percentile: 25 th (Normal) Weight 12.6lbs Weight Percentile: 14 th (Closely watch, on low side) Normal weight Recommended Calories per day: EER = (89 x weight [kg] -100) + 56 (89 X 5.72) + 56 = 565.08 kcal/day BEE = 247 − 2.67 × age (years) + 401.5 × height (meters) + 8.6 × weight (kg) = 247-2.67 X 0.4+401.5 X0.6096+8.6 X 5.715264 = 540 Fluid Formula: (0-10kg) (100Xkg), 100X5.72=572 mL/kg/day

5 + Food Frequency Questionnaire How often does the baby eat oatmeal? She just started once or twice a day in between afternoon feedings. Does the baby take any vitamin drops? Yes, Vitamin D, four times a day, once with every feeding How many ounces of formula does the baby get at a feeding? 5 to 6 ounces How often does the baby drink water? Never alone, only mixed with the powder formula. How often do you breastfeed? Never Does the baby ever take puree food? Not until 5 months as the doctor recommended How often does the baby consume Soy Formula? Always The answers given by the baby’s mother agree to the 24-Hour recall, however she forgot to include the Vitamin D drops in the 24-hour recall.

7 + Analysis of 24-hour Recall and Foodworks

8 + Analysis of the 24-hour recall and comparison to the DRI The baby’s calorie needs are lower than the range of the calculation done. The baby is consuming 513 calories. The calculations and FoodWorks recommendation ranged from 520 to 572. According to the Foodwork analysis this baby consumed less then 70% for Vitamin D, but I could not add the Vitamin D drops to Foodworks. She is taking an additional 40mcg of Vitamin D a day; therefor she is taking 46.75mcg a day, which exceeds the Upper Intake Levels. The four Vitamin D drops a day are what put her over the UL. She is also over in Zinc, this is due to the formula she is taking each day.

9 + Nutrition Diagnosis and Intervention The recommended daily calories are 520 to 572 and she consumed only 460 that day. I would recommend add another oatmeal serving, mom stated that some days she has two oatmeal servings a day to increase calories. The baby was extremely high in Vitamin D as evidenced by the 24 hour recall and food frequency questionnaire. An intervention would be remove the Vitamin D drops completely, or switch to one ML per day. Start puree food at five months when recommended by the doctor and the less formula more puree will help the balance.
