symptoms Can you see evidence of any of the following? Neologisms Blunted affect Catatonic behaviour Speech/thought disturbance Paranoid ideation Grandiose delusions

Client # 3 Joshua is a 26 year old white male. He states that he firmly believes that God talks to people and that sometimes he hears God talking directly to him. Joshua believes that God can read his mind and that he knows all of Joshua’s thoughts, feelings and beliefs. Joshua says that he has done some very bad things and that the only way that he can be forgiven is to drink the blood and eat the body of God. Although Joshua freely admits that he has never actually seen or touched God, he knows that God is real and that God loves him. He says that he knows God has a special purpose for him and that he will know what that purpose is when God decides to unveil his plans.

Would you diagnose Joshua as having a delusional disorder? Why or why not? The DSM-IV sates that when experiences are shared and accepted by members of a cultural group, they are not to be considered as evidence of a delusional disorder. Why would DSM-IV need to make this exception? What are the implications of this exception? What aspects of your culture might someone from another culture view as delusional?

Client # 4 You are a high school guidance counsellor. Harold is an 18 year old Native American student. His teacher refers him to you because Harold has been talking about visions he has seen and spirits that he talks with about many different things. His teacher became concerned about Harold after hearing him tell some friends that he was talking to the spirits of the dead, who were instructing him to go off into the mountains by himself. Harold says he often talks to these spirits and receives guidance and comfort from their presence.

1. What more would you want to know before making a diagnosis? 2. What diagnosis would you make? Why? 3. Would you diagnosis be different if Harold was White? Why?

When you interview Martha, she tells you that she is being followed by people in black cars, and that her friends are not really her friends but robots disguised to fool her. She says she sees bugs on her arms and asks for a razor to cut the bugs off. Her parents report she has never behaved this way before, but they have been concerned about her for a while. They report she recently has been hanging out with a different group of friends and she parties a lot. The have noticed that Martha doesn’t appear to sleep much.
