7. So what we did…. Families experiences within particular areas were often really good, but when support was required across more than one team, generally the family experience was not so good. Work on the Disabled Children and Adults (DCA) review started 2011 to improve this Engaged with 150 staff across social care (children and adults), local authority education services and health services Engaged with 80 customers face to face and received 240 returned electronic surveys Wiltshire applied to become a pathfinder in August 2011. Multiagency bid. Wiltshire one of 20 successful applications Asked to ‘pathfind’ on single assessment process and personal budgets The work of the DCA review and pathfinder has merged, now the DCA Pathfinder

Co-production with design team Agreement to design integrated service that brings together elements of children and adult social care, education and some targeted services to support children and young people with disabilities and Special Educational Needs aged 0 to 25 (stability) Development of new paperwork to facilitate the single assessment process and plan Development of a Special Educational Needs and Disability (SEND) Lead Worker

He finds any change really difficult and is going to be 18 next year. I was really pleased when we were asked to be involved in this project. At our meeting there was someone from education and a social worker., including one from adult care This has never happened before. We are planning what happens now and for Dave’s future. I liked the idea that all the people were in the same room and I didn’t have to say things to different groups of people.
