2. POLICY Facility shall have at least the following committees: Resident Care Infection Control Pharmaceutical Quality Assurance Safety Plan or par of Quality Assurance Plan Committee Budget Committee Utilization Review Committee (optional)

3. COMMITTEES Resident Care Committee Infection Control / Safety Safety Plan Infection Control Committee Resident Safety Plans

4. COMMITTEES -2 Pharmaceutical Services Committee Establish, review and approve policies and procedures monitoring for safe procurement, storage, distribution of drugs and biologicals Review and actions Recommend improvement of services Quality Assurance Committee Meet at least quarterly Responsible for identifying, acting upon issues affecting quality of care and services

SAFETY PLAN & COMMITTEE (see also Infection Control) Safety Committee or Safety Review process as part of another committee Reporting system for resident safety events Process for resident staff to conduct thorough analyses on resident safety events Process supports and encourages safety and reporting resident safety events Providing ongoing resident safety training

6. SAFETY PLAN & COMMITTEE (see also Infection Control) -2 Definition of a resident safety event, which shall include, but not be limited to, all adverse events or potential adverse events that are determined to be preventable and health-care associated infections. See also Infection Control Committee.

7. RESIDENT SAFETY COMMITTEE Review and approve the resident safety plan Receive and review reports of resident safety events Develop, implement, evaluate and monitor of preventive strategies Monitor implementation of corrective actions Make recommendations to eliminate future resident safety events

8. RESIDENT SAFETY COMMITTEE -2 Review and revise resident safety plans Evaluate tubes used Safety event defined (6 adverse categories): Product/device; Protection; Care management; Environmental; Criminal; Surgical (usually not in a skilled nursing facility)
