3 3 Overview Purpose/Goal:  To increase enabling services in order to facilitate access to primary health care services for special populations served by organizations funded under: Section 330(g) Migrant Health Centers; Section 330(h) Health Care for the Homeless; and/or Section 330(i) Public Housing Primary Care

4 4 Funding HRSA plans to award the following:  50 grants totaling $5 million* *Funding contingent on final Congressional appropriation $100,000/yr in both Year 1 and Year 2; Budget MAY NOT exceed CAP; Must submit two 12-month operating budgets, regardless of the # of months remaining in the project period of the parent grant; and Second year funding based on availability of funds.

5 5 Anticipated Announcement of Award  On or around September 1, 2009

6 6 Eligibility Requirements  Applicant IS an existing Health Center Program grantee that receives funding under one or more of the following: Migrant Health Centers (MHC) (Section 330(g)) Health Care for the Homeless (HCH) (Section 330(h)) Public Housing Primary Care (PHPC) (Section 330(i))

7 7 Eligibility Requirements  Applicants proposes to EXPAND enabling services to special populations in one or more of the following areas: case management; referrals; translation/interpretation; transportation; eligibility assistance; health education; environmental health risk reduction (e.g., educational materials, nicotine gum/patches); and outreach.

8 8 Eligibility Requirements  Applicant adheres to the 80-page limit on the length of the total application when printed by HRSA, whether submitting electronically or on paper (where a waiver has been granted).  Applicant has submitted a request for annual Federal section 330 funding in Year 1 and Year 2 as presented in Budget Form 424A that DOES NOT exceed the established annual cap of $100,000 in Year 1 and Year 2.  Applicant DOES NOT request funding under Section 330(e), Community Health Center Program.

9 9 Eligibility Requirements  Applicant DOES NOT propose to use Federal funding for: Adding a new service delivery site; Expenses related to land or facility purchase, construction/alteration or renovation of a facility, or any other facility related expenses that would cause a Federal interest to be placed on land or a facility;

10 10 Eligibility Requirements  Applicant DOES NOT propose to use Federal funding for : Any direct patient care and/or services including the provision of patient care services (e.g., physical exams, therapy sessions, pharmaceuticals, etc.); Any service expansions (e.g., extended clinic hours, expanded clinical services, etc.); and Major capital equipment used for reimbursable patient care and services (e.g., radiology equipment, ambulances, etc.).

11 11 Other Eligibility Information for Multiple-Funded section 330 Applicant Organizations  Organizations that currently receive Federal funds from more than one special population (HCH, PHPC, and/or MHC) MAY propose to serve one or more currently funded special population.  When applying for funding for special populations discuss how the particular enabling services will expand access for each proposed target population (i.e., migrant seasonal farmworkers, homeless clients, and/or residents of public housing) served.

12 12 Definition of Enabling Services  Non-clinical services that DO NOT include direct patient services that enable individuals to access health care and improve health outcomes. Enabling services include: case management; referrals; translation/interpretation; transportation; eligibility assistance; health education; environmental health risk reduction (e.g., educational materials, nicotine gum/patches), and outreach.

13 13 Funding Preference Sparsely Populated - Applicants may request a funding preference if they meet the definition of a sparsely populated rural or frontier area for their entire proposed service area (must provide supporting documentation). Definition of Sparsely Populated: The entire proposed area has 7 or less persons per square mile (must provide evidence to support). When determining whether the service area is sparsely populated, the entire, defined service area for the application must be considered in whole (e.g., all of the census tracts/zip codes for the entire service area, not just a specified few non-contiguous census tracks/zip codes within the specified service areas.

14 14 Application Due Dates  Applicants are required to submit some application materials in Grants.gov and HRSA EHBs. Below are the different submission due dates. This is a new 2 tier-process. Grants.gov due date– March 2, 2009 EHB due date – March 16, 2009

15 15 Electronic Submission in Grants.gov (Due Date March 2, 2009) APPLICANTS SHOULD REGISTER EARLY Registrations expire annually Process may take up to one month Applicants will submit the following in Grants.gov:  SF 424 Face Page;  Project Summary/ Abstract (uploaded on line 15 of the SF 424 Face Page); and  PHS-5161 HHS checklist.

16 16 Electronic Submission in Grants.gov Helpful Hints:  The earlier Grants.gov application is submitted, the more time applicants have to work in EHB.  Once the Grants.gov submission is complete, applicants will receive an email confirmation that they may begin completing the remainder of the application in EHB. ** Allow up to seven (7) business days for email notification.

17 17 Electronic Submission in EHB (Due date March 16, 2009) Applicants will submit the following in EHB:  SF 424A - Budget Information (Non-Construction Programs);  Program Narrative;  Budget Justification and Narrative;  SF-424B Assurances – Non-Construction Programs;  SF-424 LLL Disclosure of Lobbying Activities as applicable

18 18 Electronic Submission in EHB  Program Specific Forms—  1A – General Information Worksheet  1B – BPHC Funding Request  5-Part A – Services Provided  12 – Organization Contacts These forms will be filled out electronically online  2 – Staffing Profile  4 – Community Characteristics These forms will be completed and uploaded as an attachment.

20 20 Electronic Submission Reminders:  HRSA will NOT accept paper applications without prior approval from DGP.  SF 424 face sheet does NOT have to be printed, signed, scanned and attached/uploaded with submission.
