2  29.1 million people in the US have diabetes, with an estimated 86 million with pre-diabetes  Higher risk of blindness, kidney failure, heart disease, stroke and lower-limb amputations  People with diabetes spend 2.3 times more on health care costs than those without diabetes  7 th leading cause of death in the US UIC- Midwest Latino Health Research, Training and Policy Center

3  The percent of adults living with diagnosed diabetes for Wichita County is 11% - US Health & Human Services 2005-2011  Among our clients 32% reported at least one member in the household that has diabetes – HIA 2014 Food Bank Report  The total civilian population without health insurance coverage…19% - US Census Bureau ◦ 31% of WIC clients indicated they did not have any insurance or any way to pay for healthcare – WIC client survey, 2015  Among citizens in Wichita County, 23% are functionally illiterate – North Texas Area United Way & Wichita Adult Literacy Council

8  Empowerment DSME ◦ Focuses on building skills that motivate and empower people with diabetes to make small changes in their lives that will lead to a better life with diabetes  Traditional DSME ◦ Traditional, lecture-based classes where a person acquires knowledge but may not focus on goal- setting for behavior changes

9  Developed by the Midwest Latino Health Research, Training and Policy Center at the University of Illinois at Chicago  Training of facilitators targets community health workers  Educational curriculum designed to engage community residents in self-management practices for prevention and control of diabetes  Very interactive and fun program UIC- Midwest Latino Health Research, Training and Policy Center

Diabetes and Exercise  Managing Diabetes through Nutrition  Diabetes Complications: Identification and Prevention  Learning about Medications and Medical Care  Living with Diabetes: Mobilizing your Family and Friends UIC- Midwest Latino Health Research, Training and Policy Center

11  Making group rules  Demonstrations  Discussions  Group learning activities and games  Brief lectures using visuals  Problem-solving activities  Practice: blood glucose self-monitoring, exercise UIC- Midwest Latino Health Research, Training and Policy Center

12  6 – 10 weekly educational sessions conducted by a trained and certified Peer Trainer  Session duration: 1.5-2 hours  Monthly follow-up encouraged: support groups, informational meetings; referrals to other health programs  Encourages involvement of family and friends of participants with diabetes UIC- Midwest Latino Health Research, Training and Policy Center

13  County classes completed to date – 5  Participants who completed the program - 50  Classes in progress – 3 (2 English, 1 Spanish…38 participants)  DEEP Lunch and Learn – Last Thursday of the month at the Health District  Trained 10 additional educators in Wichita County (July 14-16, 2015)

15  Identifying the signs and warnings that mean I should get medical help  Knowing enough about diabetes to make choices that are good/right for me  Knowing how to manage personal health  Feeling motivated to do what’s necessary to manage diabetes  Can count on family members, friends or neighbors to help with diabetes care.
