3 Unique Identifiers are the Optimum Patient ID Strategy Simple Permanent, but replaceable if necessary Concise, can be incorporated into data sets Cost-effective Globally unique Flexible, works with variety of systems & methods Available Capacity Directly involves the patient Minimal impact on existing systems 3

4 Current Patient Identification Infrastructure is Inadequate EMPI matching 8% to 12% error rate Many known problems –Duplicate names –Siblings/twins –Misspellings –Marriage –Foreign names –Large populations Over 100,000 deaths annually due to errors in patient identification 4

5 Identification Errors are a Patient Safety Issue False positives create a clear patient danger False negatives mean duplicate records and needed information may not be available Additional (duplicate) testing is expensive and can be dangerous Good clinical decision support requires accurate and comprehensive information Time spent looking for information is time not spent diagnosing and treating the patient 5

7 7 Privacy Requirements Mandate Two Types of Identifiers VUHID identifiers are either Open or Private Open Voluntary Identifier – OVID –Patient identity is known –Information is available to all of a patient’s caregivers –Clinical, administrative, financial and other uses Private Voluntary Identifier – PVID –Many different classes –Patient identity is anonymous –Only three entities know the patient’s identity Patient, selected physician(s), HIE EMPI system –Specific set of “rules” for each privacy class An individual should have one OVID but may have many PVIDs

9 Privacy Considerations VUHID system is not a privacy threat –No names, demographics, clinical data Privacy classes represent the infrastructure for the ‘next generation’ of privacy management –Multiple concurrent models –Clinical data segementation –Evolution –Patient control –Complements ‘trust mechanisms’ e.g. DirectTrust –Leverage anonymity wherever feasible/desirable 9
