7 Making Work Part of the Plan Employment in MN Choices Person Centered Interview Employment Module Certified Assessor Training

8 Person Centered Interview Questions about family work history, hobbies, interests, volunteer work, education, past work history Framed in a way to elicit conversational responses

9 Assessment- Supporting Employment at All Stages Doesn’t know/believe work is an option Exploring work as an option Limits work potential (TWP, SGA, …) Wants to work to potential (no limits) Has a work plan Working as planned Work/life changes (raise, promotion, marriage, disability)

10 Employment Module Completed by everyone 13-65 Questions interest in employment, job satisfaction Identification of Potential Barriers to Employment

11 Certified Assessor Training All assessors will receive training for the new MN Choices assessment process This includes employment specific training Assessor training will help integrate employment into additional lead agency business areas

12 Referrals List of employment resources will be provided to people who complete an assessment

13 Benefits Planning System MN’s Disability Linkage Line & Disability Benefits 101 (DB101) subsystem offer person-centered information, resources and tools Help people with disabilities: better understand benefits and work incentives explore work as an option make informed choices about work achieve better employment outcomes
