4 Good Practice All commissioned services accept referrals via one route SPE hosted by Coventry and Warwickshire Partnership NHS Trust. Referrals screened by clinician/practitioners who make a decision re most appropriate service across tiers Clinicians/practitioners across 4 tiers Self referrals are accepted Professional referrals are accepted Step up and down across tiers

8 Targeted support for Looked After (LAC) adopted children and young people, their parents/carers, and associated professionals (e.g. Social Workers). Services and support for children and young people with low to moderate emotional wellbeing. Targeted tier 2 Service – Vulnerable/at risk Children

10 Reach Model Counselling: Reach will provide counselling to c&yp using their preferred method of delivered. Either Face to Face or via Therapeutic Group Work: Programmes include; Low Mood, Anger Awareness, Anxiety Management, Separation & Loss and Bereavement. Peer support: Opportunity for c&yp to manage their issues through peer support via: Online forums & Group sessions Self-help resources / information Signposting and linking to other services Online self-help resources Online information

11 The staff team Journeys and Reach employ a team with a wide range of skills and qualifications including: Primary Mental Health Workers Occupational Therapists Counsellors Youth workers NNEB Psychologists (outsourced)

14 Access to support before crisis point – making sure people with mental health problems can get help 24 hours a day and that when they ask for help, they are taken seriously. Access to support before crisis point Urgent and emergency access to crisis care – making sure that a mental health crisis is treated with the same urgency as a physical health emergency. Urgent and emergency access to crisis care Quality of treatment and care when in crisis – making sure that people are treated with dignity and respect, in a therapeutic environment.
