State Investment: HMG HV Help Me Grow Home Visiting Restructured program gets to expectant moms sooner (implemented July 16, 2010) – Program eligibility: First time expectant parents Evidence-based curricula for prenatal education – NFP: Evidence shows program impacts birth outcomes – PAT: Curriculum for prenatal weekly

State Investment: HMG HV Help Me Grow Home Visiting Strong emphasis on medical home – Connecting women to prenatal care is a program activity & expected outcome – Encourage/help connect to quality medical homes for both mother and baby – Every Child Succeeds has shown evidence of like program success

State Investment: HMG HV Help Me Grow Home Visiting Consistent access & care data collected, integrated into regular home visits – From beginning, parents and home visitors talk about access to care, quality of care, necessity of prenatal & well child care – Each home visit integrates conversation about needed referrals for mom and baby

State Investment: HMG HV How does HMG work to prevent Infant Mortality? – Education to parents & all caregivers on proper infant care, SIDS risk reduction, tobacco cessation – Safety Checklist for environmental risks/dangers – Assessments for child health, can access PHN for any medical concerns to co-visit & assess – Assessments for parents include risk of abuse/neglect

Additional Funds: MIECHV 5 years of funding from HRSA & ACF Health is lead, 8 counties have opportunity Will provide funds for at risk communities HFA or NFP, rigorous outcomes evaluation To create an effective, comprehensive EC system that supports the lifelong health and well-being of children, parents and caregivers
