2 Form Updates Part 1: Performance Planning Available in a fillable pdf format Once you complete certain fields, they default eliminating need to re-enter information Core Responsibilities from job description can be completed on Performance Plan & will default to Performance Evaluation Supervisor & Employee develop measures to assess performance

3 Performance Plan Supervisor & Employee develop learning goals along with steps needed to be taken to accomplish goals. For each performance cycle the Plan is reviewed, signed & remains in department for assessment during that performance cycle.

4 Part 2: Performance Evaluation Identification Information does not need to be entered. Defaults from Part 1: Performance Plan Core Competencies are evaluated by supervisor indicating level of achievement. Additional comments are not required.

5 Core Responsibilities Core Responsibilities default from Part 1: Performance Plan. Additional comments can be added, if appropriate. The Level of Achievement in each Core Responsibility is evaluated Summary Section for Strengths & Suggested Improvements comments.

6 Overall Rating Comment section for Development and Learning Goals. Comment section for Significant achievements &/or other results. Overall Rating is indicated. Reviewer must review prior to evaluation meeting with employee. Employee may provide comments on form or additional sheet if necessary. Part 1 (signed in 2014) and Part 2 are turned in to Human Resources by deadline of October 26, 2015.

7 Questions ????? If the Performance Plan (signed in 2014) was completed on the previous form, can I use only Part 2: Performance Evaluation of the new revised form? Yes, you can complete Part 2: on the revised form and attach the Performance Plan completed & signed in 2014 on the previous form to Human Resources. Can I use the revised form and re-enter the Performance Plan from the previous form? Yes, but you will still need to submit the previous form. It will have the original signatures dated at the beginning of the performance cycle. When is the 2015 – 16 Performance Plan returned to HR? It will be turned in with Part 2: Performance Evaluation in October, 2016. When are completed Performance Evaluations due in Human Resources? Completed Performance Evaluation packets are due no later than October 26, 2015. The packet includes the Performance Plan and the Performance Evaluation.
