3 Using Decision Support Assessment Assessment results can be linked to suggest appropriate care Care Element By breaking down care plans to smaller elements you can build a more highly individualised care plan Review By associating a formal outcome and review period you can formalise and automate the review process

4 Example 1 Braden Scale Braden Scale Result = High Risk Care Element Pressure Ulcer Risk High – this element will detail specific interventions for residents at high risk of pressure ulcers Outcome The Outcome can be set to require review at any interval e.g. 1 week to document if the outcome (prevention of pressure damage) has been achieved. If not a Variance is recorded

5 Example 2 Hearing Assessment Hearing assessment indicates resident wears a hearing aid Care Element The linked care element specifically outlines care for a resident who uses a hearing aid. Outcome Periodic review to ensure care is achieving the required outcome that the resident is able to hear.

6 Decision Support So how can this be achieved? First start by looking at your care plans Identify plans that cover a multiple of care situations and break them down into smaller care items

7 Breaking it down eg Typically, care plans for hearing cover both residents who require and use aids, don’t require aids or require aids but won’t use them Each of these could form a separate specific care item which can be linked to the applicable answer in a hearing assessment

8 Activity Sets Activity Set Activities 1 or more (Interventions) Outcomes By using an activity set you can break down care elements and apply a frequency to items that require one. Ultimately this allows for electronic sign off. On the next page I have broken down an example of a hearing deficit

9 Breaking it Down Hearing Deficit – Wears Hearing Aids Apply aids daily Frequency – Daily 0800 Check batteries daily Daily 0800 Store hearing aid in container in top drawer Daily – 2000 Resident able to hear and communicate adequately Review Frequency - Monthly

10 Putting it all together Mrs Morgan has a Braden Pressure Risk Assessment which identifies she’s at high risk due to her incontinence. The assessment has a linked care element to be added to her plan
