2 Session Overview To raise awareness of the different pharmacy teams working for STH community services. What the role entails and how we link with community pharmacy.

3 STH Community Services Take referrals from a variety of sources Who the teams are! Active Recovery (CICS) Short term intervention up to 4 weeks Community Stroke Service / IC Beds 12 week service Integrated Care Team Long term conditions

4 All teams have pharmacists and pharmacy technicians Domiciliary visits to provide pain review, fall review, disease management & adherence reviews. Team Roles Active Recovery (CICS) Admission avoidance/facilitate early hospital discharge Community Stroke Service / IC Beds Re-ablement Integrated Care Team Long term condition management

5 Ethel is admitted with fall & #NOF, has MDS, eye drops, inhalers & district nurses administering insulin Hospital pharmacy notify community pharmacy of admission Discharged to IC bed for rehab –medication changed (TTO sent to IC beds) community pharmacy supply medication and MAR Discharged home with Active Recovery CICS on medication administration (copy of current MAR chart from IC beds) Active Recovery CICS pharmacy team review Ethel. Pharmacist completes pain and falls review Pharmacy tech completes adherence review MDS to restart with prompt auto dropper supplied district nurses administer insulin. Community pharmacy contacted. Ethel discharge from Active Recovery self medicating using MDS and auto dropper with district nurses administering insulin ICT pharmacy team monitor adherence and pain Patient Journey from Hospital to Home

6 Why the patients need your help Compliance aids for reasonable adjustment to maintain independence e.g. easy open tops MURs Knowledge about the patient Emergency supplies NMS MAR charts
